If loaded with isapnp = 0 the driver explodes. This is catching
people out now and then. What should happen in the working case is
a complete mystery and the code appears terminally confused, but we
can at least make the error path work properly.

Treat both negative and zero return values from clk_round_rate() as
errors. This is needed since subsequent patches will convert
clk_round_rate()'s return value to be an unsigned type, rather than a
signed type, since some clock sources can generate rates higher than
(2^31)-1 Hz.
Eventually, when calling clk_round_rate(), only a return value of zero
will be considered a error. All other values will be considered valid
rates. The comparison against values less than 0 is kept to preserve
the correct behavior in the meantime.

"s390/time,vdso: convert to the new update_vsyscall interface"
the new update_vsyscall function already does the sum of xtime
and wall_to_monotonic. The old update_vsyscall function only
copied the wall_to_monotonic offset. The vdso code needs to be
modified to take this into consideration.

The code to use the ECTG instruction to calculate the cputime for the
current thread is currently used only for the per-thread CPU-clock
with the clockid -2 (PID=0, VIRT=1). Use the same code for the clockid
CLOCK_THREAD_CPUTIME_ID to speed up the more common clockid as well.

The access-list entry is supposed to have the fetch-only bit set, however
a reserved bit got set instead.
Userspace isn't able to write to the page anyway since the accessed page
has the read-only bit set. So this saves us only for bad surprises in the
future if the reserved bit gets used.

In current models, maximum number of active cores is 101.
[heiko.carstens@de.ibm.com]: Xose's patch increased the maximum possible
value of CONFIG_NR_CPUS to 101. I changed this to 256 instead.

struct sclp_cpu_info contains entries only for 255 cpus, while the new
smp fallback sigp detection code will fill up to 256 entries.
Even though there is no machine available which has 256 cpus and where
in addition the fallback sigp cpu detection code will be used we better
fix this, to prevent out of bound accesses.

Commit "s390/sclp: Consolidate early sclp init calls to sclp_early_detect()"
(7b50da53f6ad2048241bef232bfc22a132a40283) replaced the sclp_event_mask_early()
with sclp_set_event_mask(). The early_event_mask_sccb variable is no longer
initialized but is still used in sclp_has_linemode() and sclp_has_vt220().
Replace early_event_mask_sccb with the sccb_early variable in both
functions.

After the call to del_gendisk, the gendisk still holds a reference to
its request_queue. We must not modify the gendisks queue pointer
before the put_disk call, or the gendisk_release function cannot
release the reference and the memory for the request_queue structure
is lost.

The current logic sets the kdump base to min of 2G or ppc64_rma_size/2.
On PowerNV kernel the first memory block 'memory@0' can be very large,
equal to the DIMM size with ppc64_rma_size value capped to 1G. Hence on
PowerNV, kdump base is set to 512M resulting kdump to fail while allocating
paca array. This is because, paca need its memory from RMA region capped
at 256M (see allocate_pacas()).
This patch lowers the kdump base cap to 128M so that kdump kernel can
successfully get memory below 256M for paca allocation.

I have recently found out that no iommu_groups could be found under
/sys/ on a P8. That prevents PCI passthrough from working.
During my investigation, I found out there seems to be a missing
iommu_register_group for PHB3. The following patch seems to fix the
problem. After applying it, I see iommu_groups under
/sys/kernel/iommu_groups/, and can also bind vfio-pci to an adapter,
which gives me a device at /dev/vfio/.

The bestcomm driver has been moved to drivers/dma, so to select
this driver by default additionally CONFIG_DMADEVICES has to be
enabled. Currently it is not enabled in the config despite existing
CONFIG_PPC_BESTCOMM=y in the config files. Fix it.

The pseudo palette color entries need to be ajusted for little
endian.

The "screen" properties : depth, width, height, linebytes need
to be converted to the host endian order when read from the device
tree.
The offb_init_palette_hacks() routine also made assumption on the
host endian order.

In pte_alloc_one(), pgtable_page_ctor() is passed an address that has
not been converted by page_address() to the newly allocated PTE page.
When the PTE is freed, __pte_free_tlb() calls pgtable_page_dtor()
with an address to the PTE page that has been converted by page_address().
The mismatch in the PTE's page address causes pgtable_page_dtor() to access
invalid memory, so resources for that PTE (such as the page lock) is not
properly cleaned up.
On PPC32, only SMP kernels are affected.
On PPC64, only SMP kernels with 4K page size are affected.
This bug was introduced by commit d614bb041209fd7cb5e4b35e11a7b2f6ee8f62b8
"powerpc: Move the pte free routines from common header".
On a preempt-rt kernel, a spinlock is dynamically allocated for each
PTE in pgtable_page_ctor(). When the PTE is freed, calling
pgtable_page_dtor() with a mismatched page address causes a memory leak,
as the pointer to the PTE's spinlock is bogus.
On mainline, there isn't any immediately obvious symptoms, but the
problem still exists here.

A kernel configured with PPC_EARLY_DEBUG_BOOTX=y but PPC_PMAC=n and
PPC_MAPLE=n will fail to link:

Some boards seem to have garbage in the upper
16 bits of the vram size register. Check for
this and clamp the size properly. Fixes
boards reporting bogus amounts of vram.

Just flushing out my pile of bugfixes, most of them for regressions/cc:
stable. Nothing really serious going on.
For outstanding issues we still have the S4 fun due to the hsw S4
duct-tape pending (seems like I need to switch into angry maintainer mode
on that one). And there's the mode merging revert to make my g33 work
again still pending for drm core. For that one I don't have any more clue
(and it looks like no one else has a good idea either). And apparently the
locking WARN fix in here also needs to be replicated for boot, still
confirming that one though.

During the vmap() routine for the dma-buf, we first grab the pages and
then try to allocate a temporary array to pass to the vmap(). However,
the shrinker can and will reap any object that is unbound if the
allocation for the array first fails. This includes the object which we
are attempting to vmap(). The solution is to mark the object's pages as
pinned whilst we try the allocation to prevent the use-after-free
introduced by the potential shrinkage.

The MI_PREDICATE_RESULT_2 register exits only on HSW. On other
platforms the same offset is either reserved, or contains some
other register. So write the register only on HSW.

The ring scratch pages don't have a PPGTT mapping, so the DERRM SRM
should target the global GTT instead.

When I submitted the first patch adding these force wake functions,
Chris Wilson observed that I was using the wrong functions, so I sent
a second version of the patch to correct this problem. The problem is
that v1 was merged instead of v2.
I was able to notice the problem when running the
debugfs-forcewake-user subtest of pm_pc8 from intel-gpu-tools.
