Instead of looping all jobs and releasing their semaphores separately,
do just one semaphore release. All the jobs are using the same sema
index, and the final, maximum value of it is known.
Move also this resetting into ch->sync->set_min_eq_max() to be
consistent with syncpoints.

Semaphores don't need to be released from CPU anymore, so clarify the
code by deleting nvgpu_semaphore_release() and refactoring
__nvgpu_semaphore_release() to nvgpu_semaphore_reset() that only
"fast-forwards" the semaphore to a later value.
While doing this, the meaning of nvgpu_semaphore_incr() changes, so
rename it to nvgpu_semaphore_prepare(). Now it's only used to prepare an
nvgpu_semaphore for a value that the HW will increment the sema to.
Also change the BUG_ON that guards sema double-inits into just WARN_ON.

nvgpu_semaphore_pool_alloc() returns an ERR_PTR instead of NULL which
the caller checks on failure. Common code should not use ERR_PTRs
though, so modify nvgpu_semaphore_pool_alloc() to return error code
separately and fix nvgpu_init_sema_pool() to consider this.

struct nvgpu_semaphore represents (mainly) a threshold value that a sema
at some index will get and struct nvgpu_semaphore_int (aka "hw_sema")
represents the allocation (and write access) of a semaphore index and
the next value that the sema at that index can have. The threshold
object doesn't need a pointer to the sema allocation that is not even
guaranteed to exist for the whole threshold lifetime, so replace the
pointer by the position of the sema in the sema pool.
This requires some modifications to pass a hw sema around explicitly
because it now represents write access more explicitly.
Delete also the index field of semaphore_int because it can be directly
derived from the offset in the sema location and is thus unnecessary.

Compare gpu semaphores in the kernel in the same way as the hardware
does: released if value is over threshold, but at most half of u32's
range. This makes it possible to skip zeroing the sema values when semas
are allocated, so that they'd be just monotonically increasing numbers
like syncpoints are.

The hw semas in a sema pool are stored in a list. All elements in this
list are freed in a loop when a semaphore pool is destroyed. However,
each hw sema is always owned by a channel, and each such channel frees
its hw sema during channel closure before putting a ref to the VM which
holds a ref to the sema pool, so the lifetime of all the hw semas is
shorter than that of the pool and this list is always empty when freeing
the pool. Delete the list and this freeing loop.
Meanwhile delete also the nr_incrs member in nvgpu_semaphore_int that is
never accessed.

This change solves two problems:
(*) the possibility of a crash due to interrupting the gpu
initialization following a bind
(*) a IOVA memory leak that could prevent the GPU from binding after
about 200 bind/unbind cycles
A detailed list of fixes:
- chek that arbiter is initialized before freeing it.
- do not re-enable interrupts when MSI is enabled on unbind.
- free the semaphore sea on unbind.
- ensure we dont double load the vbios.
- check return value of nvgpu_mutex_init for semaphores.
- add corresponding nvgpu_mutex_destroy calls.

Use struct nvgpu_mem for DMA allocations (and the corresponding
nvgpu_dma_alloc_sys()) instead of custom rolled code. This migrates
away from using linux scatter gather tables directly. Instead this
is hidden in the nvgpu_mem struct. With this change the semaphore.c
code no longer has any direct Linux dependencies.

Update the debugging APIs in nvgpu to be more coherent and less dependent
on Linux kernel APIs and data structures. The old API is maintained for
backwards compatibility but the prints themselves are changed.
The old API now prints stuff like this:

Each line starts with 'nvgpu:' since this is the nvgpu driver. Then there's
the function name which is is right justified and post-fixed with a line
number. This keeps all statements lined up so long as the function name does
not exceed the length of characters alloted for function names (33 currently).
Lines are also left justified with 4 available spaces since there are currently
no source files with over 9999 lines. The type of message (DBG, WRN, ERR) is
then printed in brackets. Finally there's whatever message is to be printed.
The new API will be largely the same except there will be an additional
print of the GPU that the message has originated from in order to help debug
cases where both an iGPU and dGPU are active. Also the implicit newline
added in the legacy API has been removed. This is inline with essentially
every other print function in the C world. There are numerous places where
'\n' is added despite it not being needed which results in log files being
littered with blank lines. This makes the logs much harder to read.

Instead of using Linux APIs for mutex and spinlocks
directly, use new APIs defined in <nvgpu/lock.h>
Replace Linux specific mutex/spinlock declaration,
init, lock, unlock APIs with new APIs
e.g
struct mutex is replaced by struct nvgpu_mutex and
mutex_lock() is replaced by nvgpu_mutex_acquire()
And also include <nvgpu/lock.h> instead of including
<linux/mutex.h> and <linux/spinlock.h>
Add explicit nvgpu/lock.h includes to below

Move semaphore_gk20a.c drivers/gpu/nvgpu/common/ since the semaphore
code is common to all chips.
Move the semaphore_gk20a.h header file to drivers/gpu/nvgpu/include/nvgpu
and rename it to semaphore.h. Also update all places where the header
is inluced to use the new path.
This revealed an odd location for the enum gk20a_mem_rw_flag. This should
be in the mm headers. As a result many places that did not need anything
semaphore related had to include the semaphore header file. Fixing this
oddity allowed the semaphore include to be removed from many C files that
did not need it.
