Adds the skeleton and integration of the GV100 endpoint driver to NVGPU
(1) Adds a OS abstraction layer for the internal nvlink structure.
(2) Adds linux specific integration with Nvlink core driver.
(3) Adds function pointers for nvlink api, initialization and isr process.
(4) Adds initial support for minion.
(5) Adds new GPU enable properties to handle NVLINK presence
(6) Adds new GPU enable properties for SG_PHY bypass (required for NVLINK over
PCI)
(7) Adds parsing of nvlink vbios structures.
(8) Adds logging defines for NVGPU

Move fields in struct gk20a related to interrupt handling into
Linux specific nvgpu_os_linux. At the same time move the counter
logic from function in HAL into Linux specific code, and two Linux
specific power management functions from generic gk20a.c to Linux
specific module.c.

Move non-function pointer members out of the mc substruct of
gpu_ops. Ideally gpu_ops will have only function ponters, better
matching its intended purpose and improving readability.
gops.mc.intr_mask_restore is now mc_intr_mask_restore

Reorganize HAL initialization to remove inheritance and construct
the gpu_ops struct at compile time. This patch only covers the mc
sub-module of the gpu_ops struct.
Perform HAL function assignments in hal_gxxxx.c through the
population of a chip-specific copy of gpu_ops.

Split handling of stalling interrupt to Linux specific chip
agnostic and OS independent chip specific parts.
Linux specific chip independent part contains handler for ISR
and passing the control to a bottom half worker. It uses the new MC
HALs intr_nonstall (query interrupt status), intr_nonstall_pause
(pause interrupts), intr_nonstall_resume (resume interrupts), and
is_intr1_pending (query per-engine interrupt bit).
MC HAL isr_nonstall is removed, because its work is now handled in
chip independent code.

Add MC HAL is_intr1_pending. At the same time introduce nvgpu_unit
that is passed as parameter to is_intr1_pending. The API is passed
contents of intr1 register and an engine number, and returns true
if there's an interrupt pending for the engine.

Add wrapper header nvgpu/types.h. It checks if build is for Linux
kernel and pulls in linux/types.h.
Delete also all #includes for linux/types.h which are not strictly
necessary, and change the remaining ones to use the new wrapper.

Clean up gk20a.c by removing direct accesses to MC and moving the
accesses to happen via MC HAL.
The chip detection logic has to violate the HAL and call gk20a
version directly, because HAL ops cannot be set up before chip has
been identified.

Currently priv, pbus and ltc interrupts are enabled as non-stall but
being handled in stall isr. Fix is to configure them as stall interrupt.

(*) Refactor the non-stalling interrupt path to execute clear on the
top half, so on dGPU case processing of stalling interrupts does not
block non-stalling one.
(*) Use a worker thread to do semaphore wakeups and allow batching of
the non-stalling operations.
(*) Fix a bug where some gpus will not properly track the completion
of interrupts, preventing safe driver unloads

-Handle pbus and priv stall interrupts first.
In general critical interrupts should be
handled before any other non critical ones.
-Dump info enabled with gpu_dbg_intr if priv_ring
interrupt is flagged by fmodel.

Move the gp10b HW headers to a new directory specially for them:
include/nvgpu/hw/gp10b
And change the code to include like so:
#include <nvgpu/hw/gp10b/hw_fb_gp10b.h>
This is part of the process to restructure the nvgpu driver.

Implement a helper function engine_enum_from_type. This allows
parsing device_info entries for LCE engine type.
Pascal has logical copy engine instead of CE2, so so add definition
of that.
