Added sw method support for SET_BES_CROP_DEBUG4.
In this sw method:
CLAMP_FP_BLEND_TO_MAXVAL forces overflow and
CLAMP_FP_BLEND_TO_INF blend results to clamp to FP maxval.
Added support for this sw method in gp10b/gp106/gv11b
and gv100.

Move graphics context ownership to TSG instead of channel. Combine
channel_ctx_gk20a and gr_ctx_desc to one structure, because the split
between them was arbitrary. Move context header to be property of
channel.

Used chip specific attrib_cb_gfxp_default_size and
attrib_cb_gfxp_size buffer sizes during committing
global callback buffer when gfx preemption is requested.
These sizes are different for gv11b from gp10b.
For gp10b used smaller buffer sizes than specified
value in hw manuals as per sw requirement.
Also used gv11b specific preemption related functions:
gr_gv11b_set_ctxsw_preemption_mode
gr_gv11b_update_ctxsw_preemption_mode
This is required because preemption related buffer
sizes are different for gv11b from gp10b. More optimization
will be done as part of NVGPU-484.
Another issue fixed is: gpu va for preemption buffers
still needs to be 8 bit aligned, even though 49 bits
available now. This done because of legacy implementation
of fecs ucode.

This reverts commit caf168e33ec12ff6f0ed90fd4aa7654c09eaa553.
Might be causing an intermittency in quill-c03 graphics submit. Super
weird since the only change that seems like it could affect it is the
header file update but that seems rather safe.

With recent rework in nvgpu most of the <uapi/linux/nvgpu.h> includes
are not needed so remove them
Remove use of NVGPU_DBG_GPU_REG_OP_* in gk20a/gr_gk20a.c and use common
definition instead
Remove use of NVGPU_ALLOC_GPFIFO_FLAGS_REPLAYABLE_FAULTS_ENABLE in
gp10b/fifo_gp10b.c by defining new common flag
NVGPU_GPFIFO_FLAGS_REPLAYABLE_FAULTS_ENABLE and then parsing it in API
nvgpu_gpfifo_user_flags_to_common_flags()

We use linux specific graphics/compute preemption modes defined in uapi header
(and of below form) in all over common code
NVGPU_GRAPHICS_PREEMPTION_MODE_*
NVGPU_COMPUTE_PREEMPTION_MODE_*
Since common code should be independent of linux specific code, define new modes
of the form in common code and used them everywhere
NVGPU_PREEMPTION_MODE_GRAPHICS_*
NVGPU_PREEMPTION_MODE_COMPUTE_*
Add required parser functions to convert both the modes into each other
For linux IOCTL NVGPU_IOCTL_CHANNEL_SET_PREEMPTION_MODE, we need to convert
linux specific modes into common modes first before passing them to common code
And to pass gpu characteristics to user space we need to first convert common
modes into linux specific modes and then pass them to user space

Reorganize HAL initialization to remove inheritance and construct
the gpu_ops struct at compile time. This patch only covers the
gr sub-module of the gpu_ops struct.
Perform HAL function assignments in hal_gxxxx.c through the
population of a chip-specific copy of gpu_ops.

The new SET_BES_CROP_DEBUG3 sw method is used to flip two fields
in the NV_PGRAPH_PRI_BES_CROP_DEBUG3 register. The sw method is
used by the user space driver to disable enough ROP optimizations
to maintain ZBC state of target tiles.

gk20a_err() and gk20a_warn() require a struct device pointer,
which is not portable across operating systems. The new nvgpu_err()
and nvgpu_warn() macros take struct gk20a pointer. Convert code
to use the more portable macros.

Rename the nvgpu DMA APIs from gk20a_gmmu_alloc* to nvgpu_dma_alloc*.
This better reflects the purpose of the APIs (to allocate DMA suitable
memory) and avoids confusion with GMMU related code.

This ioctl can be used on gp10b to set a flag in the context header
indicating this context should be run at elevated clock
frequency. FECS ctxsw ucode will read this flag as part of the context
switch and will request higher GPU clock frequencies from BPMP for the
duration of the context execution.

Move the gp106 HW headers to a new directory specially for them:
include/nvgpu/hw/gp106
And change the code to include like so:
#include <nvgpu/hw/gp106/hw_fb_gp106.h>
This is part of the process to restructure the nvgpu driver.

Discrete GPU does not have clocks wired correctly. They're needed to
be able to calculate correct preemption timeout, so disable setting
the timeout.
