We use SZ_* #defines in some parts of nvgpu, but we don't explicitly
include a header that defines it. Add include/nvgpu/sizes.h that in
Linux #includes linux/sizes.h.

Add an alignment check for compressible-kind fixed-address
mappings. If we're using page size smaller than the comptag line
coverage window, the GPU VA and the physical buffer offset must be
aligned in respect to that window.

Since NVGPU_AS_MAP_BUFFER_FLAGS_DIRECT_KIND_CTRL was made mandatory,
kernel does not need to know the details about the PTE kinds
anymore. Thus, we can remove the kind_gk20a.h header and the code
related to kind table setup, as well as simplify buffer mapping code
a bit.

gk20a_err() and gk20a_warn() require a struct device pointer,
which is not portable across operating systems. The new nvgpu_err()
and nvgpu_warn() macros take struct gk20a pointer. Convert code

Move all programming of FB to fb_*.c files, and remove the inclusion
of FB hardware headers from other files.
TLB invalidate function took previously a pointer to VM, but the new
API takes only a PDB mem_desc, because FB does not need to know about
higher level VM.
GPC MMU is programmed from the same function as FB MMU, so added
dependency to GR hardware header to FB.
GP106 ACR was also triggering a VPR fetch, but that's not applicable
to dGPU, so removed that call.

On Maxwell comptaglines are assigned per 128k, but preferred big page
size for graphics is 64k. Bit 16 of GPU VA is used for determining
which half of comptagline is used.
This creates problems if user space wants to map a page multiple times
and to arbitrary GPU VA. In one mapping the page might be mapped to
lower half of 128k comptagline, and in another mapping the page might
be mapped to upper half.
Turn on mode where MSB of comptagline in PTE is used instead of bit 16
for determining the comptagline lower/upper half selection.

Implement support for 64kB large page size. Add an API to create an
address space via IOCTL so that we can accept flags, and assign one
flag for enabling 64kB large page size.
Also adds APIs to set per-context large page size. This is possible
only on Maxwell, so return error if caller tries to set large page
size on Kepler.
Default large page size is still 128kB.
