Currently, the kind map initialization inherits from earlier
chips' kind map definitions and simply adds on newly supported
features. This is dangerous as changes in older architectures
may affect newer architectures in unpredictable ways.
This patch removes inheritance between subsequent architectures'
kind map initializations.

Kind value can be passed to API nvgpu_vm_map() from User
space (through IOCTL NVGPU_AS_IOCTL_MAP_BUFFER_EX)
But kind value is not checked for sane values
before storing it in bfr.kind_v
And then we use this kind value as array index
in gk20a_kind_is_supported() which is incorrect
Fix this by ensuring in nvgpu_vm_map() that the
kind value is well within range

Remove use of linux/kernel.h and linux/compiler.h. We don't use
anything in those headers.
Also replace use of linux/bitops.h with new wrapper nvgpu/bitops.h.

Reorganize the HW headers of gk20a. The headers are moved to a
new directory:
include/nvgpu/hw/gk20a
And from the code are included like so:
#include <nvgpu/hw/gk20a/hw_pwr_gk20a.h>
This is the first step in reorganizing all of the HW headers for
gm20b, gm206, etc. This is part of a larger effort to re-structure
and make the driver more readable and scalable.

If code coverage is enabled on GCC 4.7, the kernel build fails in
gk20a_init_kind_attr() since GCC decides to inline almost everything in
this file into it, leading to a massive stack frame with over kilobyte's
worth of temporary variables generated by gcov, leading to this error:
kind_gk20a.c: In function 'gk20a_init_kind_attr':
kind_gk20a.c:424:1: error: the frame size of 1232 bytes is larger than
1024 bytes [-Werror=frame-larger-than=]
(Just removing the inline keyword doesn't work, as GCC still decides to
inline it, so noinline_for_stack is actually required.)
