Implement a worker thread to replace the delayed works in channel
watchdog and job cleanups. Watchdog runs by polling the channel states
periodically, and job cleanup is performed on channels that are appended
on a work queue consumed by the worker thread. Handling both of these
two in the same thread makes it impossible for them to cause a deadlock,
as has previously happened.
The watchdog takes references to channels during checking and possibly
recovering channels. Jobs in the cleanup queue have an additional
reference taken which is released after the channel is processed. The
worker is woken up from periodic sleep when channels are added to the
queue.
Currently, the queue is only used for job cleanups, but it is extendable
for other per-channel works too. The worker can also process other
periodic actions dependent on channels.
Neither the semantics of timeout handling or of job cleanups are yet
significantly changed - this patch only serializes them into one
background thread.
Each job that needs cleanup is tracked and holds a reference to its
channel and a power reference, and timeouts can only be processed on
channels that are tracked, so the thread will always be idle if the
system is going to be suspended, so there is currently no need to
explicitly suspend or stop it.

Instead of using Linux APIs for mutex and spinlocks
directly, use new APIs defined in <nvgpu/lock.h>
Replace Linux specific mutex/spinlock declaration,
init, lock, unlock APIs with new APIs
e.g
struct mutex is replaced by struct nvgpu_mutex and
mutex_lock() is replaced by nvgpu_mutex_acquire()
And also include <nvgpu/lock.h> instead of including
<linux/mutex.h> and <linux/spinlock.h>

In the PCI init/de-init code only disable IRQs if they were enabled.
The enable happens in the common power_on code. The PCI routines could
both be called without the common power_on code being called which
causes an extra disable to happen.

Remove gm204/gm206 support. It was used only in the interim until
Pascal cards were available, and we don't maintain that code anymore.

Most of BIOS parsing code is not specific to any particular GPU. Move
most of the code to generic files, and leave only chip specific parts
dealing with microcontroller boot into chip specific files.
As most of the parsing is generic, they do not need to be called via
HALs so remove the HALs and change the calls into direct function
calls.
All definitions meant to be used outside BIOS code itself are now in
<nvgpu/bios.h>

This ioctl can be used on gp10b to set a flag in the context header
indicating this context should be run at elevated clock
frequency. FECS ctxsw ucode will read this flag as part of the context
switch and will request higher GPU clock frequencies from BPMP for the
duration of the context execution.

Make the GPU bind and rebind operations work when the driver
is idle. This required two changes.
1. Reset the GPU before doing SW init for PCI GPUs. This clears
the SW state which may be stale in the case of a rebind attempt.
2. Cleanup the interrupt enable/disables. Firstly there was one
place where nvgpu would accidentally disable the stalling
interrupt twice when the stalling interrupt and non-stalling
interrupt are the same. Secondly make sure when exiting nvgpu
that the interrupt enable/disables are balanced. Leaving the
interrupt in the -1 disable state means that next time the
driver runs interrupts never quite get enabled.

Add a full GPU reset function to the XVE block. This allows
the driver to reset the GPU (except the XVE and XP interfaces)
to clear the GPU's state.
This is necessary for the GPU rebind to work. The state of the
GPU needs to be cleared before the new driver instance can work.

Use the timers API in the gk20a code instead of Linux specific
API calls.
This also changes the behavior of several functions to wait for
the full timeout for each operation that can timeout. Previously
the timeout was shared across each operation.

Handle possible asynchronous GPU driver shutdown more gracefully.
This occurs when the GPU disappears from the PCI bus, for example,
if it overheats or detects an over current event.
Also add a preprocessor check to make sure that the
gk20a_channel_cancel_pending_sema_waits()
is always defined. In some builds CONFIG_SYNC is disabled but the
gk20a_remove_support() code does not check for this.

In many helper functions like gk20a_readl() the code assumed that
the GPU is present and registers and available. However, during
GPU shutdown this may not be the case.
In theory the driver should not be accessing GPU registers during
GPU shutdown (since shutdown is triggered by GPU registers being
unavailable) but these changes handle any missed cases where this
may happen.
This goes for GPU device access as well. Many parts of the code
assume that if the struct gk20a is valid, the the GPU dev must be
there are well. This isn't always the case, it seems.

Process long regops lists in 4-kB fragments, overcoming the overly

- PG statistics read support for multiple engines
- updated stat_dmem_offset member to array to hold
dmem offset of PG engines
- PMU allocates memory in DMEM for each PG engine requested,
updated gk20a_pmu_get_elpg_residency_gating() to get
engine statistics for requested PG engine

- Added enable_mscg, mscg_enabled & mscg_stat flags,
mscg_enabled flag can be used to controll
mscg enable/disable at runtime along with mscg_stat flag.
- Added defines & interface to support ms/mclk-change/post-init-param
- Added defines for lpwr tables read from vbios.
- HAL to support post init param which is require
to setup clockgating interface in PMU & interfaces used during
mscg state machine.
- gk20a_pmu_pg_global_enable() can be called when pg support
required to enable/disable, this also checks & wait
if pstate switch is in progress till it complets
- pg_mutex to protect PG-RPPG/MSCG enable/disable

Store pending sema waits so that they can be explicitly handled when
the driver dies. If the sema_wait is freed before the pending wait is
either handled or canceled problems occur.
Internally the sync_fence_wait_async() function uses the kernel timers.
That uses a linked list of possible events. That means every so often
the kernel iterates through this list. If the list node that is in the
sync_fence_waiter struct is freed before it can be removed from the
pending timers list then the kernel timers list can be corrupted. When
the kernel then iterates through this list crashes and other related
problems can happen.

Check if the GPU is present after each register read. If the a register
read returns 0xffffffff then it's possible the GPU has fallen off the
bus for some reason or another. However, to confirm that a register read
is due to a dead GPU vs just a 0xffffffff being returned by happenstance
the chip ID register is read which should never return 0xffffffff. If
that read returns 0xffffffff as well then certainly the GPU is dead.

Add HAL interface for TSG open, which is intended to
be called from the exisiting gk20a_tsg_open function.
The tsg_open entryoint is only implemented for vgpu,
as the server needs to clear metadata when a tsg is opened.

In gk20a_scale_target(), to check for duplicate
freq requests we compare current frequency with
devfreq->previous_freq
But for very first request after boot, we have
devfreq->previous_freq set to MIN freq
And in case we evaluate new frew as MIN freq
then we skip calling postscale() and scaling
of EMC clock
This results in keeping EMC at MAX value
To fix this, add new variable last_freq in
gk20a structure.
Use this variable to store frequency value
and to compare for duplicate requests
