Refactor the sync_debugfs LTC HAL op so that the logic to enable
or disable LTC goes to common code nvgpu_ltc_sync_enabled() and
the LTC HAL set_enabled only performs the hardware register access.
Create a new common function nvgpu_init_ltc_support() to initialize
the LTC software variable, and move hardware initialization of LTC to
be called from it.

In some cases page directories require less than a full page of memory.
For example, on Pascal, the final PD level for large pages is only 256 bytes;
thus 16 PDs can fit in a single page. To allocate an entire page for each of
these 256 B PDs is extremely wasteful. This patch aims to alleviate the
wasted DMA memory from having small PDs in a full page by packing multiple
small PDs into a single page.
The packing is implemented as a slab allocator - each page is a slab and
from each page multiple PD instances can be allocated. Several modifications
to the nvgpu_gmmu_pd struct also needed to be made to support this. The
nvgpu_mem is now a pointer and there's an explicit offset into the nvgpu_mem
struct so that each nvgpu_gmmu_pd knows what portion of the memory it's
using.
The nvgpu_pde_phys_addr() function and the pd_write() functions also require
some changes since the PD no longer is always situated at the start of the
nvgpu_mem.
Initialization and cleanup of the page tables for each VM was slightly
modified to work through the new pd_cache implementation. Some PDs (i.e
the PDB), despite not being a full page, still require a full page for
alignment purposes (HW requirements). Thus a direct allocation method for
PDs is still provided. This is also used when a PD that could in principle
be cached is greater than a page in size.
Lastly a new debug flag was added for the pd_cache code.

To support deterministic channels even with platforms where railgating
is supported, have each deterministic-marked channel hold a power
reference during their lifetime, and skip taking power refs for jobs in
submit path for those.
Previously, railgating blocked deterministic submits in general because
of gk20a_busy()/gk20a_idle() calls in submit path possibly taking time
and more significantly because the gpu may need turning on which takes a
nondeterministic and long amount of time.
As an exception, gk20a_do_idle() can still block deterministic submits
until gk20a_do_unidle() is called. Add a rwsem to guard this. VPR resize
needs do_idle, which conflicts with deterministic channels' requirement
to keep the GPU on. This is documented in the ioctl header now.
Make NVGPU_GPU_FLAGS_SUPPORT_DETERMINISTIC_SUBMIT_NO_JOBTRACKING always
set in the gpu characteristics now that it's supported. The only thing
left now blocking NVGPU_GPU_FLAGS_SUPPORT_DETERMINISTIC_SUBMIT_FULL is
the sync framework.
Make the channel debug dump show which channels are deterministic.

Split handling of stalling interrupt to Linux specific chip
agnostic and OS independent chip specific parts.
Linux specific chip independent part contains handler for ISR
and passing the control to a bottom half worker. It uses the new MC
HALs intr_nonstall (query interrupt status), intr_nonstall_pause
(pause interrupts), intr_nonstall_resume (resume interrupts), and
is_intr1_pending (query per-engine interrupt bit).
MC HAL isr_nonstall is removed, because its work is now handled in
chip independent code.

Support only VM pointers and ref-counting for maintaining VMs. This
dramatically reduces the complexity of the APIs, avoids the API
abuse that has existed, and ensures that future VM usage is
consistent with current usage.
Also remove the combined VM free/instance block deletion. Any place
where this was done is now replaced with an explict free of the
instance block and a nvgpu_vm_put().

Begin removing all of the myriad flag variables in struct gk20a and
replace that with one API that checks for flags being enabled or
disabled. The API is as follows:
bool nvgpu_is_enabled(struct gk20a *g, int flag);
bool __nvgpu_set_enabled(struct gk20a *g, int flag, bool state);
These APIs allow many of the gk20a flags to be replaced by defines.
This makes flag usage consistent and saves a small amount of memory in
struct gk20a. Also it makes struct gk20a easier to read since there's
less clutter scattered through out.

nvgpu_log/info/warn/err() internally add a \n to the end of the message.
Hence, callers should not include a \n at the end of the message. Doing
so results in duplicate \n being printed, which ends up creating empty
log messages. Remove the duplicate \n from all err/warn messages.

Add these bits in the gpu characteristics flags:
NVGPU_GPU_FLAGS_SUPPORT_DETERMINISTIC_SUBMIT_NO_JOBTRACKING - fast
submits with no in-kernel job tracking are supported.
NVGPU_GPU_FLAGS_SUPPORT_DETERMINISTIC_SUBMIT_FULL - deterministic
submits also with job tracking and num_inflight_jobs set are supported.
Either of these may get disabled if the particular channel or submit
still requires features that block these.
Make gk20a_channel_sync_needs_sync_framework() take a gk20a pointer
instead of a channel pointer so that it can be called without a channel.
It does not need any per-channel data.

Add nvgpu_* wrappers for determining if we're running in simulation
or silicon, and if we're running in hypervisor.
The new wrappers require struct gk20a pointer, and gk20a_fence_wait()
did not have access to one. Add struct gk20a pointer as the first
parameter.

In gk20a_pm_shutdown(), we do not check return value
of gk20a_pm_prepare_poweroff
In some cases it is possible that gk20a_pm_prepare_poweroff()
returns -EBUSY (this could happen if engines are busy)
so we don't clean up s/w state and directly
trigger GPU railgate
In case some interrupt is triggered simultaneously
we try to access a register while GPU is already railgated
This leads to a hard hang in nvgpu shutdown path
Make below changes in shutdown sequence to fix this:
- check return value of gk20a_wait_for_idle()
- disable activity on all engines with
gk20a_fifo_disable_all_engine_activity()
- ensure engines are idle with gk20a_fifo_wait_engine_idle()

gk20a_err() and gk20a_warn() require a struct device pointer,
which is not portable across operating systems. The new nvgpu_err()
and nvgpu_warn() macros take struct gk20a pointer. Convert code
to use the more portable macros.

Update the debugging APIs in nvgpu to be more coherent and less dependent
on Linux kernel APIs and data structures. The old API is maintained for
backwards compatibility but the prints themselves are changed.
The old API now prints stuff like this:

Each line starts with 'nvgpu:' since this is the nvgpu driver. Then there's
the function name which is is right justified and post-fixed with a line
number. This keeps all statements lined up so long as the function name does
not exceed the length of characters alloted for function names (33 currently).
Lines are also left justified with 4 available spaces since there are currently
no source files with over 9999 lines. The type of message (DBG, WRN, ERR) is
then printed in brackets. Finally there's whatever message is to be printed.
The new API will be largely the same except there will be an additional
print of the GPU that the message has originated from in order to help debug
cases where both an iGPU and dGPU are active. Also the implicit newline
added in the legacy API has been removed. This is inline with essentially
every other print function in the C world. There are numerous places where
'\n' is added despite it not being needed which results in log files being
littered with blank lines. This makes the logs much harder to read.

On unbind we need to check that interrupts are complete before tearing
down the interrupt threads, but on vgpu those structures are not
initialized as they are managed by the server.
This change makes sure we do not try to free those resources on vgpu
shutdown

fifo reset_enable_hw is reorged to clear and enable pbdma/fifo interrupts
after all the required configuration such as configuring timeouts,

Clean up gk20a.c by removing direct accesses to MC and moving the
accesses to happen via MC HAL.
The chip detection logic has to violate the HAL and call gk20a
version directly, because HAL ops cannot be set up before chip has
been identified.

This change refactors the teardown in remove to ensure that it is
possible to unload the driver while leaving fds open. This is achieved
by making sure that the SW state is kept alive till all fds are closed
and by checking that subsequent calls to ioctls after the teardown fail.
Normally, this would be achieved ny calls into gk20a_busy(), but in
kickoff we dont call into that to reduce latency, so we need to check
the driver status directly, and also in some of the functions
as we need to make sure the ioctl does not dereference the device or
platform struct

After driver remove, the device structure passed in gk20a_busy can be
invalid. To solve this the prototype of the function is modified to pass
the gk20a struct instead of the device pointer.

Add CONFIG_TEGRA_GK20A_NVHOST and remove the TEGRA_GRHOST ||
TEGRA_HOST1X dependency in CONFIG_TEGRA_GK20A to allow using the iGPU
without the nvhost driver. Use the new config to guard syncpt-related
code.
Also make TEGRA_ACR depend on GK20A too so that it aligns properly under
gk20a in menuconfig.
