Switch all logging to nvgpu_log*(). gk20a_dbg* macros are
intentionally left there because of use from other repositories.
Because the new functions do not work without a pointer to struct
gk20a, and piping it just for logging is excessive, some log messages
are deleted.

When using a coherent DMA API wee must make sure to program
any aperture fields with the coherent aperture setting. To
do this the nvgpu_aperture_mask() function was modified to
take a third aperture mask argument, a coherent setting, so
that code can use this function to generate coherent aperture
settings.
The aperture choice is some what tricky: the default version
of this function uses the state of the DMA API to determine
what aperture to use for SYSMEM: either coherent or
non-coherent internally. Thus a kernel user need only specify
the normal nvgpu_mem struct and the correct mask should be
chosen. Due to many uses of nvgpu_mem structs not created
directly from the DMA API wrapper it's easier to translate
SYSMEM to SYSMEM_COH after creation.
However, the GMMU mapping code, will encounter buffers from
userspace with difference coerency attributes than the DMA
API. Thus the __nvgpu_aperture_mask() really respects the
aperture setting passed in regardless of the DMA API state.
This aperture setting is pulled from NVGPU_VM_MAP_IO_COHERENT
since this is either passed in from userspace or set by the
kernel when using coherent DMA. The aperture field in attrs
is upgraded to coh if this flag is set.
This change also adds a coherent sysmem mask everywhere that
it can. There's a couple places that do not have a coherent
register field defined yet. These need to eventually be
defined and added.
Lastly the aperture mask code has been mvoed from the Linux
vm.c code to the general vm.c code since this function has
no Linux dependencies.

channel_ctx_gk20a and gr_ctx_desc to one structure, because the split
between them was arbitrary. Move context header to be property of

Split the ctxsw trace "core" API code into <nvgpu/ctxsw_trace.h>. This
is not perect though since there's some Linuxisms present in the HAL
and as such that code has to be hidden by the ctxsw tracing CONFIG. But
this patch should work for QNX such that it will allow the code to
build as long as CONFIG_GK20A_CTXSW_TRACE is not set.
Also fix the copywrite notice in the ctxsw code present under
common/linux to be GPL.

Move much of the remaining generic MM code to a new common location:
common/mm/mm.c. Also add a corresponding <nvgpu/mm.h> header. This
mostly consists of init and cleanup code to handle the common MM
data structures like the VIDMEM code, address spaces for various
engines, etc.
A few more indepth changes were made as well.
1. alloc_inst_block() has been added to the MM HAL. This used to be
defined directly in the gk20a code but it used a register. As a
result, if this register hypothetically changes in the future,
it would need to become a HAL anyway. This path preempts that
and for now just defines all HALs to use the gk20a version.
2. Rename as much as possible: global functions are, for the most
part, prepended with nvgpu (there are a few exceptions which I
have yet to decide what to do with). Functions that are static
are renamed to be as consistent with their functionality as
possible since in some cases function effect and function name
have diverged.

This change solves crashes during bind that were introduced in the driver
during the OS unification refactoring due to lack of coverage of the remove()
function.
The fixes during remove are:
(1) Prevent NULL dereference on GPUs with secure boot
(2) Prevent NULL dereferences when fecs_trace is not enabled
(3) Added PRAMIN blocker during driver removal if HW is no longer accesible
(4) Prevent double free of debugfs nodes as they are handled on the
debugfs_remove_recursive() call
(5) quiesce() can now be called without checking is HW accesible flag is set
(6) added function to free irq so no IRQ association is left on the driver after
it is removed
(7) prevent NULL dereference on nvgpu_thread_stop() if the thread is already
stopped

Reorganize HAL initialization to remove inheritance and construct
the gpu_ops struct at compile time. This patch only covers the
fecs_trace sub-module of the gpu_ops struct.
Perform HAL function assignments in hal_gxxxx.c through the
population of a chip-specific copy of gpu_ops.

Put all debugfs dependencies inside #ifdef CONFIG_DEBUG_FS. This
includes some functions in allocators that were used only for
debugging.
Remove include of linux/debugfs.h on files that do not deal with
debugfs.
linux/debugfs.h implicitly included linux/fs.h, which we relied on.
Add explicit include of linux/fs.h for all files where this is the
case.

FECS trace used hrtimer for sleeping between frames. The resolution
does not need to be that high, and hrtimers are not portable, so
replace the delay with nvpgu_usleep_range().

For dGPU, instance block is in vidmem, and context_ptr
was not properly computed, leading to reporting pid=0 in
FECS traces.
Use gk20a_mm_inst_block_addr, which handles all cases
to determine instance block physical address.

gk20a_err() and gk20a_warn() require a struct device pointer,
which is not portable across operating systems. The new nvgpu_err()
and nvgpu_warn() macros take struct gk20a pointer. Convert code
to use the more portable macros.

Rename the nvgpu DMA APIs from gk20a_gmmu_alloc* to nvgpu_dma_alloc*.
This better reflects the purpose of the APIs (to allocate DMA suitable
memory) and avoids confusion with GMMU related code.

Use the new kmem API functions in misc gk20a code. Some additional
modifications were also made:
o Add a struct gk20a pointer to gk20a_fence to enable proper
kmem free usage.
o Add gk20a pointer to alloc_session() in dbg_gpu_gk20a.c to
use kmem API for allocating a session.
o Plumb a gk20a pointer through the fence creation and deletion.
o Use statically allocated buffers for names in file creation.

After driver remove, the device structure passed in gk20a_busy can be
invalid. To solve this the prototype of the function is modified to pass
the gk20a struct instead of the device pointer.

Added fecs trace characteristic flag to vgpu to determine for
platform support
Also moved fecs flag from ctxsw init to fecs init for native linux.
This makes flag init uniform across both platforms.

Instead of using Linux APIs for mutex and spinlocks
directly, use new APIs defined in <nvgpu/lock.h>
Replace Linux specific mutex/spinlock declaration,
init, lock, unlock APIs with new APIs
e.g
struct mutex is replaced by struct nvgpu_mutex and
mutex_lock() is replaced by nvgpu_mutex_acquire()
And also include <nvgpu/lock.h> instead of including
<linux/mutex.h> and <linux/spinlock.h>
Add explicit nvgpu/lock.h includes to below
files to fix complilation failures.

Currently, when reading from ctxsw device node, we are collecting
traces that occurred before enabling tracing. This is not wanted,
and makes testing unpredicatable.
This change drops existing data in FECS ring buffer when enabling
traces, as currently done on vm-server side.

Use the timers API in the gk20a code instead of Linux specific
API calls.
This also changes the behavior of several functions to wait for
the full timeout for each operation that can timeout. Previously
the timeout was shared across each operation.

Reorganize the HW headers of gk20a. The headers are moved to a
new directory:
include/nvgpu/hw/gk20a
And from the code are included like so:
#include <nvgpu/hw/gk20a/hw_pwr_gk20a.h>
This is the first step in reorganizing all of the HW headers for
gm20b, gm206, etc. This is part of a larger effort to re-structure
and make the driver more readable and scalable.

Flushing timestamp record method can fail in case FECS is not
processing the main method queue. In particular, this occurs
in case of ctxsw timeout, where we process fifo sched interrupts
from the host, but FECS is still waiting for idle (grWFI).
In such scenario, this adds huge delay in fifo recovery
procedure (timeout on FECS method). Since flushing the last
(incomplete) record from FECS would only be useful in that case
(context switch ongoing), remove flush operation on engine
reset. Note that an explicit ENGINE_RESET event (with pid)
is inserted in user-facing ctxsw buffer on engine reset.

When processing FECS traces, a hash table is used
to retrieve the 'pid' of the process that created
the channel/TSG. Report process identifer (aka
tgid in kernel) instead of thread identifier (aka
pid) for FECS traces.

app manager to control TSG scheduling parameters:
- Get list of TSGs
- Get list of recent TSGs

For devices that have vidmem available, use the vidmem allocator in
gk20a_gmmu_alloc{,attr,_map,_map_attr}. For others, use sysmem.
Because all of the buffers haven't been tested to work in vidmem yet,
rename calls to gk20a_gmmu_alloc{,attr,_map,_map_attr} to have _sys at
the end to declare explicitly that vidmem is used. Enabling vidmem for
each now is a matter of removing "_sys" from the function call.
