Add API for querying reset mask corresponding to a unit. The reset
masks need to be read from MC HW header, and we do not want all
units to access Mc HW headers themselves.

The perf inst block was being treated as vidmem (LFB - local
framebuffer) always, regardless of the type of nvgpu_mem used
for the instance block. On dGPUs this was fine becasue we
always allocate instance blocks from vidmem. Inst blocks are
allocated with nvgpu_dma_alloc() which chooses vidmem if
vidmem is present, otherwise falls back to sysmem.
When the above fall back logic was deleted this caused inst
blocks to always be allocated in sysmem, even for dGPUs. This
isn't a problem in an of itself but the logic for the perf
instance block bind operation assumed a VIDMEM inst_block.
Thus this patch uses the nvgpu_aperture_mask() function to
correctly program the required aperture target for the perf's
inst block bind operation.

In the current code, gk20a.h includes io.h which gets directly included
in a lot of other files. io.h contains methods which uses a struct
gk20a as a parameter leading to a circular dependency between io.h
and gk20a.h. This can be mitigated by removing io.h from gk20a.h as
part of larger effort to moving gk20a.h to nvgpu/gk20a.h

Switch all logging to nvgpu_log*(). gk20a_dbg* macros are
intentionally left there because of use from other repositories.
Because the new functions do not work without a pointer to struct
gk20a, and piping it just for logging is excessive, some log messages
are deleted.

The value of NV_PERF_PMASYS_MEM_BUMP is different for Volta
and NVGPU_IOCTL_CHANNEL_CYCLE_STATS_SNAPSHOT_CMD_FLUSH did not
have correct behavior on GV11B due to that.
The patch adds an instance of css_hw_set_handled_snapshots
for Volta to fix that.
The patch also renames css_hw_set_handled_snapshots
to gk20a_css_hw_set_handled_snapshots to make it more clear
that the function is arch dependent.

Data can be speculatively loaded from memory and stay in cache even
when bound check fails. This can lead to unintended information
disclosure via side-channel analysis.

We use SZ_* #defines in some parts of nvgpu, but we don't explicitly
include a header that defines it. Add include/nvgpu/sizes.h that in
Linux #includes linux/sizes.h.

In css_gr_create_client_data(), we call css_gr_free_client_data() in case
cur->perfmon_start is zero
css_gr_free_client_data() deletes the node from list, and performs some
cleanup if cur->perfmon_start is non-zero
Hence it is incorrect to call css_gr_free_client_data() in clean up path
since node is not added to the list, and cur->perfmon_start is zero anyways
Fix the cleanup path, and remove call to css_gr_free_client_data()

We right now store dmabuf fd and dma_buf pointer for gk20a_cs_snapshot_client
But since dma_buf and all related APIs are linux specific, we need to remove
them from common code and move them to linux specific code
Add new linux specific structure gk20a_cs_snapshot_client_linux which includes
struct gk20a_cs_snapshot_client and linux specific dma_buf pointer
In gk20a_attach_cycle_stats_snapshot(), we first handle all dma_buf related
operations and then call gr_gk20a_css_attach()
Move gk20a_channel_free_cycle_stats_snapshot() to ioctl_channel.c
In gk20a_channel_free_cycle_stats_snapshot(), we call gr_gk20a_css_detach()
and then free up dma_buf in linux specific code
We also need to call gk20a_channel_free_cycle_stats_snapshot() while closing
the channel, so call it from linux specific nvgpu_channel_close_linux()

Move much of the remaining generic MM code to a new common location:
common/mm/mm.c. Also add a corresponding <nvgpu/mm.h> header. This
mostly consists of init and cleanup code to handle the common MM
data structures like the VIDMEM code, address spaces for various
engines, etc.
A few more indepth changes were made as well.
1. alloc_inst_block() has been added to the MM HAL. This used to be
defined directly in the gk20a code but it used a register. As a
result, if this register hypothetically changes in the future,
it would need to become a HAL anyway. This path preempts that
and for now just defines all HALs to use the gk20a version.
2. Rename as much as possible: global functions are, for the most
part, prepended with nvgpu (there are a few exceptions which I
have yet to decide what to do with). Functions that are static
are renamed to be as consistent with their functionality as
possible since in some cases function effect and function name
have diverged.

Reorganize HAL initialization to remove inheritance and construct
the gpu_ops struct at compile time. This patch only covers the
css sub-module of the gpu_ops struct.
Perform HAL function assignments in hal_gxxxx.c through the
population of a chip-specific copy of gpu_ops.

Support only VM pointers and ref-counting for maintaining VMs. This
dramatically reduces the complexity of the APIs, avoids the API
abuse that has existed, and ensures that future VM usage is
consistent with current usage.
Also remove the combined VM free/instance block deletion. Any place
where this was done is now replaced with an explict free of the
instance block and a nvgpu_vm_put().

gk20a_err() and gk20a_warn() require a struct device pointer,
which is not portable across operating systems. The new nvgpu_err()
and nvgpu_warn() macros take struct gk20a pointer. Convert code
to use the more portable macros.

Rename the nvgpu DMA APIs from gk20a_gmmu_alloc* to nvgpu_dma_alloc*.

Use the new kmem API functions in misc gk20a code. Some additional
modifications were also made:
o Add a struct gk20a pointer to gk20a_fence to enable proper
kmem free usage.
o Add gk20a pointer to alloc_session() in dbg_gpu_gk20a.c to
use kmem API for allocating a session.
o Plumb a gk20a pointer through the fence creation and deletion.
o Use statically allocated buffers for names in file creation.

Instead of using Linux APIs for mutex and spinlocks
directly, use new APIs defined in <nvgpu/lock.h>
Replace Linux specific mutex/spinlock declaration,
init, lock, unlock APIs with new APIs
e.g
struct mutex is replaced by struct nvgpu_mutex and
mutex_lock() is replaced by nvgpu_mutex_acquire()
And also include <nvgpu/lock.h> instead of including

Reorganize the HW headers of gk20a. The headers are moved to a
new directory:
include/nvgpu/hw/gk20a
And from the code are included like so:
#include <nvgpu/hw/gk20a/hw_pwr_gk20a.h>
This is the first step in reorganizing all of the HW headers for
gm20b, gm206, etc. This is part of a larger effort to re-structure
and make the driver more readable and scalable.

For devices that have vidmem available, use the vidmem allocator in
gk20a_gmmu_alloc{,attr,_map,_map_attr}. For others, use sysmem.
Because all of the buffers haven't been tested to work in vidmem yet,
rename calls to gk20a_gmmu_alloc{,attr,_map,_map_attr} to have _sys at
the end to declare explicitly that vidmem is used. Enabling vidmem for
each now is a matter of removing "_sys" from the function call.

The cyclestats mode-e feature supported by userspace only
for t210 devices, so kernel should advertize it only for t210.
Also small check added to prevent BUG in dma-buf.c:826
if device has lack of memory.

1. In case of unacceptably small buffer passed as a cyclestats snapshot
it causes a kernel panic during list element removal:

That is a kernel supporting code for cyclestats mode E.
Cyclestats mode E implemented following Windows-design in user-space
and required the following operations to be implemented:
- attach a client for shared hardware buffer of device
- detach client from shared hardware buffer
- flush means copy of available data from hardware buffer to private
client buffers according to perfmon IDs assigned for clients
- perfmon IDs management for user-space clients
- a NVGPU_GPU_FLAGS_SUPPORT_CYCLE_STATS_SNAPSHOT capability added
