GPU hardware block needs tegra fuse clock to mirror
gpu fuses from tegra fuses to gpu domain.
Tegra fuse driver provided following APIs to
enable/disable tegra fuse clock:
int tegra_fuse_clock_enable(void);
int tegra_fuse_clock_disable(void);
To ensure that tegra fuse clock is disabled by nvgpu
driver when gpu hardware block is not in use by:
Calling tegra_fuse_clock_enable() while doing
gk20a_pm_unrailgate() and calling
tegra_fuse_clock_disable() while doing
gk20a_pm_railgate().

Move fuse override DT handling to Linux code. All the chip specific
fuse override functions did the same thing, so delete the HAL and
call the same function to read the DT overrides on all chips.
Also remove the fuse override functionality from dGPU. There are no
DT entries for PCIe devices, so it would've failed anyway.

ioctl_channel.h and cde.h referred to multiple structures that were
not forward declared or explitly #included in. Add several forward
declarations and #includes. Also add #include for
<uapi/linux/nvgpu.h> to multiple Linux .c files that were missing it.

struct gk20a is now part of nvgpu_os_linux in Linux builds. gk20a.c
still frees struct gk20a by kfree(struct gk20a *), which is wrong.
Create a new function pointer in struct gk20a for freeing the
structure and call kfree(struct nvgpu_os_linux *) instead.

Split the ctxsw trace "core" API code into <nvgpu/ctxsw_trace.h>. This
is not perect though since there's some Linuxisms present in the HAL
and as such that code has to be hidden by the ctxsw tracing CONFIG. But
this patch should work for QNX such that it will allow the code to
build as long as CONFIG_GK20A_CTXSW_TRACE is not set.
Also fix the copywrite notice in the ctxsw code present under
common/linux to be GPL.

Migrate ctxsw_trace_gk20a.c to common/linux/ctxsw_trace.c. This
has been done becasue the ctxsw tracing code is currently too
tightly tied to the Linux OS due to usage of a couple system calls:
- poll()
- mmap()
And general Linux driver framework code. As a result pulling the
logic out of the FECS tracing code is simply too large a scope for
time time being.
Instead the code was just copied as much as possible. The HAL ops
for the FECS code was hidden behind the FECS tracing config so
that the vm_area_struct is not used when QNX does not define said
config. All other non-HAL functions called by the FECS ctxsw
tracing code ha now also been hidden by this config. This is not
pretty but for the time being it seems like the way to go.

We currently check that no job is pending before proceeding with
suspend. This prevents suspend, when we could simply disable and
preempt all channels. Moreover, pending jobs accounting is done
using pm_runtime usage count, which is not updated for GPUs with
pm_runtime disabled (e.g. vgpu).
Replaced the check on pm_runtime usage count, with a check on
gk20a handle usage count. Suspend is allowed when there is no
task inside a busy/idle sequence.

We currently check that engine is idle before proceeding with
suspend. This prevents suspend when we could simply disable and
preempt all channels. Moreover, doing such a check in virtualization
case, would require to query engine status from RM server, before
proceeding with suspend.
Removed check on engine idle for system suspend.

Add abstraction of IO aperture accessors. Add new functions
gk20a_io_exists() and gk20a_io_valid_reg() to remove dependencies to
aperture fields from common code.
Implement Linux version of the abstraction by moving gk20a_readl()
and gk20a_writel() to new Linux specific io.c. Move the fields
defining IO aperture to nvgpu_os_linux.
Add t19x specific IO aperture initialization functions and add t19x
specific section to nvgpu_os_linux.

For sync-point read map:
1. Added nvgpu_mem memory allocator in gk20a struct and
allocated memory for this in gk20a_finalize_poweron()
and freed this memory in gk20a_remove().
2. Added "u64 syncpt_ro_map_gpu_va" in vm_gk20a struct
for read map in vm.
Added nvgpu_quiesce() in nvgpu_remove() before freeing
syncpoint read map to ensure that nvgpu is idle.

This change solves crashes during bind that were introduced in the driver
during the OS unification refactoring due to lack of coverage of the remove()
function.
The fixes during remove are:
(1) Prevent NULL dereference on GPUs with secure boot
(2) Prevent NULL dereferences when fecs_trace is not enabled
(3) Added PRAMIN blocker during driver removal if HW is no longer accesible
(4) Prevent double free of debugfs nodes as they are handled on the
debugfs_remove_recursive() call
(5) quiesce() can now be called without checking is HW accesible flag is set
(6) added function to free irq so no IRQ association is left on the driver after
it is removed
(7) prevent NULL dereference on nvgpu_thread_stop() if the thread is already
stopped

The following changes are part of the porting of the bind/unbind
functionality.
These changes reuse the shutdown codepaths in iGPU and dGPU and fix a locking
issue with in gk20a_busy() where the usage count can lead to a deadlock during
the driver shutdown. It fixes a racing condition with the gr/mm code by
invalidating the sw ready flag while holding the busy lock

CDE is only used in Linux platforms, and the code is highly dependent
on Linux APIs. Move the common CDE code to Linux module and leave only
the chip specific parts to HAL.

If an error occurs during an attempt to perform a runtime_resume,
the runtime power management framework sets an error flag that
prevents further attempts to resume until the error is cleared.
nvgpu currently does not clear the flag, which causes nvgpu to
lock up if an error occurs during runtime_resume.
This change explicitly sets the device pm status to suspended
on error, which clears the error flag so that subsequent attempts
to resume will not be blocked.

Remove dependency for nvgpu to invoke devfreq govenor on every
gk20a_busy/idle() call. This dependency was originally necessary to
track GPU load (busy vs idle) in software. However, since we currently
read the load GPU from HW/PMU there is no need to invoke the devfreq
governor in this path. Instead it can use timer-based polling.

Enable the log.h debug printing to print log messages on
boot based on the NVGPU_DEFAULT_DBG_MASK define. Previously
this only affected the legacy gk20a_dbg() function which
is being slowly phased out.

msleep is not recommended for (1ms - 20ms). So use usleep_range
instead to have a more deterministic sleep time.
Also fix the print for target_ref_count that could either be 2 or
1 based on whether GPU rail-gating is enabled or not.

To make do_idle work when nvgpu is built as a module, reverse the order
of call dependencies for do_idle. Don't provide visible
gk20a_do_{idle,unidle}() functions for the kernel but instead call the
kernel for registering and unregistering pointers to them when the
driver loads and unloads.
Refactor the internal __gk20a_do_{idle,unidle} functions to take a
struct gk20a * instead of struct device *, and use the callback api for
providing that g instead of retrieving the plat device from device tree.

To support deterministic channels even with platforms where railgating
is supported, have each deterministic-marked channel hold a power
reference during their lifetime, and skip taking power refs for jobs in
submit path for those.
Previously, railgating blocked deterministic submits in general because
of gk20a_busy()/gk20a_idle() calls in submit path possibly taking time
and more significantly because the gpu may need turning on which takes a
nondeterministic and long amount of time.
As an exception, gk20a_do_idle() can still block deterministic submits
until gk20a_do_unidle() is called. Add a rwsem to guard this. VPR resize
needs do_idle, which conflicts with deterministic channels' requirement
to keep the GPU on. This is documented in the ioctl header now.
Make NVGPU_GPU_FLAGS_SUPPORT_DETERMINISTIC_SUBMIT_NO_JOBTRACKING always
set in the gpu characteristics now that it's supported. The only thing
left now blocking NVGPU_GPU_FLAGS_SUPPORT_DETERMINISTIC_SUBMIT_FULL is
the sync framework.
Make the channel debug dump show which channels are deterministic.

Split handling of stalling interrupt to Linux specific chip
agnostic and OS independent chip specific parts.
Linux specific chip independent part contains handler for ISR
and passing the control to a bottom half worker. It uses the new MC
HALs intr_nonstall (query interrupt status), intr_nonstall_pause
(pause interrupts), intr_nonstall_resume (resume interrupts), and
is_intr1_pending (query per-engine interrupt bit).
MC HAL isr_nonstall is removed, because its work is now handled in
chip independent code.

Since all debugfs code is Linux specific, remove
it from common code and move it to Linux module

Add corresponding header files for above modules too
And compile all of above files only if CONFIG_DEBUG_FS is set
Some more details of the changes made
- Move and rename gk20a/debug_gk20a.c to common/linux/debug.c
- Move and rename gk20a/debug_gk20a.h to include/nvgpu/debug.h
- Remove gm20b/debug_gm20b.c and gm20b/debug_gm20b.h and call
gk20a_init_debug_ops() directly from gm20b_init_hal()
- Update all debug APIs to receive struct gk20a as parameter
instead of receiving struct device pointer
- Update API gk20a_dmabuf_get_state() to receive struct gk20a
pointer instead of struct device
- Include <nvgpu/debug.h> explicitly in all files where debug
operations are used
- Remove "gk20a/platform_gk20a.h" include from HAL files
which no longer need this include
- Add new API gk20a_debug_deinit() to deinitialize debugfs
and call it from gk20a_remove()
- Move API gk20a_debug_dump_all_channel_status_ramfc() to
gk20a/fifo_gk20a.c

Begin removing all of the myriad flag variables in struct gk20a and
replace that with one API that checks for flags being enabled or
disabled. The API is as follows:
bool nvgpu_is_enabled(struct gk20a *g, int flag);
bool __nvgpu_set_enabled(struct gk20a *g, int flag, bool state);
These APIs allow many of the gk20a flags to be replaced by defines.
This makes flag usage consistent and saves a small amount of memory in
struct gk20a. Also it makes struct gk20a easier to read since there's
less clutter scattered through out.

We use g->busy_lock in gk20a_do_idle() to prevent submitting
more jobs to h/w and to wait for currently running jobs to
finish
But requesting this lock in gk20a_idle() prevents decrementing
runtime counter and hence gk20a_do_idle() can timeout with
below prints
