Use boardobjgrp_super for boardobj_e255 and boardobj_32.
In both cases, we want the destructor to remove and destroy
related objects, as well as freeing any pmucmd buffer.

Maintain a list of boardobj and boardobjgrp, so that we can free
related objects when removing pmu support. A flag is added in
boardobj so that the destructor can determine if it should free
the object. This 'allocated' flag is false when the object is
embedded into another structure, which should be freed through
other means.

- Renamed "struct pmu_gk20a" to "struct nvgpu_pmu" then moved
to file "pmu.h" under folder "drivers/gpu/nvgpu/include/nvgpu/"
- Included header file "pmu.h" to dependent file &
removed "pmu_gk20a.h" include if its usage is not present.
- Replaced "struct pmu_gk20a" with "struct nvgpu_pmu" in dependent
source & header files.

Add nvgpu_firmware data structure, and return it instead of Linux
struct firmare from nvgpu_request_firmware. Also add abstraction
for releasing firmware: nvgpu_release_firmware.

gk20a_err() and gk20a_warn() require a struct device pointer,
which is not portable across operating systems. The new nvgpu_err()
and nvgpu_warn() macros take struct gk20a pointer. Convert code
to use the more portable macros.

When we allocate boardobj object, we assign the allocated address
to *ppboardobj. The check for result of allocation checks looks
at address in ppboardobj.
Fix the check so that it actually checks results of kzalloc().
