gpu: nvgpu: fix coverity, Recursion in included headers
- Fix recursion in included header,
gk20a.h -> perf.h -> pstate.h -> gk20a.h
- Removed "gk20a.h" from pstate.h but need to forward declare
"struct gk20a" as referenced in header file.
