Bcache needs rw semaphores for cache coherency in writeback mode -
writes have to take a read lock on a per cache device rw sem, and
release it when the bio completes.
But since this is for bios it's naturally not in the context of the
process that originally took the lock.

Now that the on-disk activity-log ring buffer size is adjustable,
the maximum active set can become larger, and is now limited by
the use of 16bit "labels".
This increases the maximum working set from 6433 to 65534 extents,
each of which covers an area of 4MiB.
Which means that if you use the maximum, you'd have to resync
more than 250 GiB after an unclean Primary shutdown.
With capable backend storage and replication links,
this is entirely feasible.

New helper to be able to consolidate more updates
into a single transaction.
Without this, we can only grab a single refcount
on an updated element while preparing a transaction.
lc_get_cumulative - like lc_get; also finds to-be-changed elements
@lc: the lru cache to operate on
@enr: the label to look up
Unlike lc_get this also returns the element for @enr, if it is belonging to
a pending transaction, so the return values are like for lc_get(),
plus:
pointer to an element already on the "to_be_changed" list.
In this case, the cache was already marked %LC_DIRTY.
Caller needs to make sure that the pending transaction is completed,
before proceeding to actually use this element.

For example,
1) DISCARD rq-1 with size size 4GB
2) DISCARD rq-2 with size size 1GB
If these 2 discard requests get merged, final request size will be 5GB.
In this case, request's __data_len field may overflow as it can store
max 4GB(unsigned int).
This issue was observed while doing mkfs.f2fs on 5GB SD card:

This is the pull request for the earlier patchset[1] with the same
name. It's only three patches (the first one was committed to
workqueue tree) but the merge strategy is a bit involved due to the
dependencies.
* Because the conversion needs features from wq/for-3.10,
block/for-3.10/core is based on rc3, and wq/for-3.10 has conflicts
with rc3, I pulled mainline (rc5) into wq/for-3.10 to prevent those
workqueue conflicts from flaring up in block tree.
* Resolving the issue that Jan and Dave raised about debugging
requires arch-wide changes. The patchset is being worked on[2] but
it'll have to go through -mm after these changes show up in -next,
and not included in this pull request.

Pulling it into block/for-3.10/core produces a conflict in
drivers/md/raid5.c between the following two commits.
e3620a3ad5 ("MD RAID5: Avoid accessing gendisk or queue structs when not available")
2f6db2a707 ("raid5: use bio_reset()")
The conflict is trivial - one removes an "if ()" conditional while the
other removes "rbi->bi_next = NULL" right above it. We just need to
remove both. The merged branch is available at
git://git.kernel.org/pub/scm/linux/kernel/git/tj/wq.git block-test-merge
so that you can use it for verification. The test merge commit has
proper merge description.
While these changes are a bit of pain to route, they make code simpler
and even have, while minute, measureable performance gain[3] even on a
workload which isn't particularly favorable to showing the benefits of
this conversion.

Writeback implements its own worker pool - each bdi can be associated
with a worker thread which is created and destroyed dynamically. The
worker thread for the default bdi is always present and serves as the
"forker" thread which forks off worker threads for other bdis.
there's no reason for writeback to implement its own worker pool when
using unbound workqueue instead is much simpler and more efficient.
This patch replaces custom worker pool implementation in writeback
with an unbound workqueue.
The conversion isn't too complicated but the followings are worth
mentioning.
* bdi_writeback->last_active, task and wakeup_timer are removed.
delayed_work ->dwork is added instead. Explicit timer handling is
no longer necessary. Everything works by either queueing / modding
/ flushing / canceling the delayed_work item.
* bdi_writeback_thread() becomes bdi_writeback_workfn() which runs off
bdi_writeback->dwork. On each execution, it processes
bdi->work_list and reschedules itself if there are more things to
do.
The function also handles low-mem condition, which used to be
handled by the forker thread. If the function is running off a
rescuer thread, it only writes out limited number of pages so that
the rescuer can serve other bdis too. This preserves the flusher
creation failure behavior of the forker thread.
* INIT_LIST_HEAD(&bdi->bdi_list) is used to tell
bdi_writeback_workfn() about on-going bdi unregistration so that it
always drains work_list even if it's running off the rescuer. Note
that the original code was broken in this regard. Under memory
pressure, a bdi could finish unregistration with non-empty
work_list.
* The default bdi is no longer special. It now is treated the same as
any other bdi and bdi_cap_flush_forker() is removed.
* BDI_pending is no longer used. Removed.
* Some tracepoints become non-applicable. The following TPs are
removed - writeback_nothread, writeback_wake_thread,
writeback_wake_forker_thread, writeback_thread_start,
writeback_thread_stop.
Everything, including devices coming and going away and rescuer
operation under simulated memory pressure, seems to work fine in my
test setup.

This is for the new bio splitting code. When we split a bio, if the
split occured on a bvec boundry we reuse the bvec for the new bio. But
that means bio_free() can't free it, hence the explicit flag.

__bio_for_each_segment() iterates bvecs from the specified index
instead of bio->bv_idx. Currently, the only usage is to walk all the
bvecs after the bio has been advanced by specifying 0 index.
For immutable bvecs, we need to split these apart;
bio_for_each_segment() is going to have a different implementation.
This will also help document the intent of code that's using it -
bio_for_each_segment_all() is only legal to use for code that owns the
bio.

This adds a pointer to the bvec array to struct bio_integrity_payload,
instead of the bvecs always being inline; then the bvecs are allocated
with bvec_alloc_bs().
Changed bvec_alloc_bs() and bvec_free_bs() to take a pointer to a
mempool instead of the bioset, so that bio integrity can use a different
mempool for its bvecs, and thus avoid a potential deadlock.
This is eventually for immutable bio vecs - immutable bvecs aren't
useful if we still have to copy them, hence the need for the pointer.
Less code is always nice too, though.
Also, bio_integrity_alloc() was using fs_bio_set if no bio_set was
specified. This was wrong - using the bio_set doesn't protect us from
memory allocation failures, because we just used kmalloc for the
bio_integrity_payload. But it does introduce the possibility of
deadlock, if for some reason we weren't supposed to be using fs_bio_set.

bip_vec in bio_integrity_payload was an array appended to the end of the
payload, so the bio_vecs in struct bio_pair should have come after the

Fix it by making bip_vec a pointer to the inline vecs - a later patch is

Previously, if we ever try to allocate more than once from the same bio
set while running under generic_make_request() (i.e. a stacking block
driver), we risk deadlock.
This is because of the code in generic_make_request() that converts
recursion to iteration; any bios we submit won't actually be submitted
(so they can complete and eventually be freed) until after we return -
this means if we allocate a second bio, we're blocking the first one
from ever being freed.
Thus if enough threads call into a stacking block driver at the same
time with bios that need multiple splits, and the bio_set's reserve gets
used up, we deadlock.
This can be worked around in the driver code - we could check if we're
running under generic_make_request(), then mask out __GFP_WAIT when we
go to allocate a bio, and if the allocation fails punt to workqueue and
retry the allocation.
But this is tricky and not a generic solution. This patch solves it for
all users by inverting the previously described technique. We allocate a
rescuer workqueue for each bio_set, and then in the allocation code if
there are bios on current->bio_list we would be blocking, we punt them
to the rescuer workqueue to be submitted.
This guarantees forward progress for bio allocations under
generic_make_request() provided each bio is submitted before allocating
the next, and provided the bios are freed after they complete.
Note that this doesn't do anything for allocation from other mempools.
Instead of allocating per bio data structures from a mempool, code
should use bio_set's front_pad.
Tested it by forcing the rescue codepath to be taken (by disabling the
first GFP_NOWAIT) attempt, and then ran it with bcache (which does a lot
of arbitrary bio splitting) and verified that the rescuer was being
invoked.

Add a flag REQ_PM to identify the request is PM related, such requests
will not change the device request queue's runtime status. It is
intended to be used in driver's runtime PM callback, so that driver can
perform some IO to the device there with the queue's runtime status
unaffected. e.g. in SCSI disk's runtime suspend callback, the disk will
be put into stopped power state, and this require sending a command to
the device. Such command processing should not change the disk's runtime
status.

Thanks to Zach Brown's work to rip out the retry infrastructure, we don't
need this anymore - ki_retry was only called right after the kiocb was
initialized.
This also refactors and trims some duplicated code, as well as cleaning up
the refcounting/error handling a bit.

Previously, allocating a kiocb required touching quite a few global
(well, per kioctx) cachelines... so batching up allocation to amortize
those was worthwhile. But we've gotten rid of some of those, and in
another couple of patches kiocb allocation won't require writing to any
shared cachelines, so that means we can just rip this code out.

Cancelling kiocbs requires adding them to a per kioctx linked list,
which is one of the few things we need to take the kioctx lock for in
the fast path. But most kiocbs can't be cancelled - so if we just do
this lazily, we can avoid quite a bit of locking overhead.
While we're at it, instead of using a flag bit switch to using ki_cancel
itself to indicate that a kiocb has been cancelled/completed. This lets
us get rid of ki_flags entirely.

Analagous to wait_event_timeout() and friends, this adds
wait_event_hrtimeout() and wait_event_interruptible_hrtimeout().
Note that unlike the versions that use regular timers, these don't
return the amount of time remaining when they return - instead, they
return 0 or -ETIME if they timed out. because I was uncomfortable with
the semantics of doing it the other way (that I could get it right,
anyways).
If the timer expires, there's no real guarantee that expire_time -
current_time would be <= 0 - due to timer slack certainly, and I'm not
sure I want to know the implications of the different clock bases in
hrtimers.
If the timer does expire and the code calculates that the time remaining
is nonnegative, that could be even worse if the calling code then reuses
that timeout. Probably safer to just return 0 then, but I could imagine
weird bugs or at least unintended behaviour arising from that too.
I came to the conclusion that if other users end up actually needing the
amount of time remaining, the sanest thing to do would be to create a
version that uses absolute timeouts instead of relative.

Freeing a kiocb needed to touch the kioctx for three things:
* Pull it off the reqs_active list
* Decrementing reqs_active
* Issuing a wakeup, if the kioctx was in the process of being freed.
This patch moves these to aio_complete(), for a couple reasons:
* aio_complete() already has to issue the wakeup, so if we drop the
kioctx refcount before aio_complete does its wakeup we don't have to
do it twice.
* aio_complete currently has to take the kioctx lock, so it makes sense
for it to pull the kiocb off the reqs_active list too.
* A later patch is going to change reqs_active to include unreaped
completions - this will mean allocating a kiocb doesn't have to look
at the ringbuffer. So taking the decrement of reqs_active out of
kiocb_free() is useful prep work for that patch.
This doesn't really affect cancellation, since existing (usb) code that
implements a cancel function still calls aio_complete() - we just have
to make sure that aio_complete does the necessary teardown for cancelled
kiocbs.
It does affect code paths where we free kiocbs that were never
submitted; they need to decrement reqs_active and pull the kiocb off the
reqs_active list. This occurs in two places: kiocb_batch_free(), which
is going away in a later patch, and the error path in io_submit_one.

This removes the retry-based AIO infrastructure now that nothing in tree
is using it.
We want to remove retry-based AIO because it is fundemantally unsafe.
It retries IO submission from a kernel thread that has only assumed the
mm of the submitting task. All other task_struct references in the IO
submission path will see the kernel thread, not the submitting task.
This design flaw means that nothing of any meaningful complexity can use
retry-based AIO.
This removes all the code and data associated with the retry machinery.
The most significant benefit of this is the removal of the locking
around the unused run list in the submission path.

The current kernel returns -EINVAL unless a given mmap length is
"almost" hugepage aligned. This is because in sys_mmap_pgoff() the
given length is passed to vm_mmap_pgoff() as it is without being aligned
with hugepage boundary.
This is a regression introduced in commit 40716e29243d ("hugetlbfs: fix
alignment of huge page requests"), where alignment code is pushed into
hugetlb_file_setup() and the variable len in caller side is not changed.
To fix this, this patch partially reverts that commit, and adds
alignment code in caller side. And it also introduces hstate_sizelog()
in order to get proper hstate to specified hugepage size.

Copy resource table from first to second firmware loading.
After firmware is loaded to memory, update the vdevs resource
pointer to the resource table kept in device memory.

Pull late ARM Exynos multiplatform changes from Arnd Bergmann:
"These continue the multiplatform support for exynos, adding support
for building most of the essential drivers (clocksource, clk, irqchip)
when combined with other platforms. As a result, it should become
really easy to add full multiplatform exynos support in 3.11, although
we don't yet enable it for 3.10.
The changes were not included in the earlier multiplatform series in
order to avoid clashes with the other Exynos updates.
This also includes work from Tomasz Figa to fix the pwm clocksource
code on Exynos, which is not strictly required for multiplatform, but
related to the other patches in this set and needed as a bug fix for
at least one board."

This series from Tomasz Figa restores support for the pwm clocksource
in Exynos, which was broken during the conversion of the platform
to the common clk framework. The clocksource is only used in one
board in the mainline kernel (universal_c210), and this makes it
work for DT based probing as well as restoring the non-DT based
case.
