changeset 04934e44e3784a1b969582e2d59afcec278470c6 removed the last implementation
that were still using the V4L1 obsoleted header.
Now, video_decoder.h is not used anymore by any driver.
Let's remove it and all references for it in Kernel.

- Now, all internal ioctls are at v4l2-common.h
- removed unused ioctl at saa6752hs.h
- all debug ioctl code moved to v4l2-common.c
- removed duplicated stuff from other cards

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
