Change the vxattr handling for getxattr so that vxattrs are checked

If we do not specifically understand a setxattr on a ceph.* virtual
xattr, send it through to the MDS. This allows us to implement new
functionality via the MDS without direct support on the client side.

ceph_calc_file_object_mapping() takes (among other things) a "file"
offset and length, and based on the layout, determines the object
number ("bno") backing the affected portion of the file's data and
the offset into that object where the desired range begins. It also
computes the size that should be used for the request--either the
amount requested or something less if that would exceed the end of
the object.
This patch changes the input length parameter in this function so it
is used only for input. That is, the argument will be passed by
value rather than by address, so the value provided won't get
updated by the function.
The value would only get updated if the length would surpass the
current object, and in that case the value it got updated to would
be exactly that returned in *oxlen.
Only one of the two callers is affected by this change. Update
ceph_calc_raw_layout() so it records any updated value.

The MDS may have incorrect wanted caps after importing caps. So the
client should check the value mds has and send cap update if necessary.

When client wants to release an imported cap, it's possible there
is no reserved cap_release message in corresponding mds session.
so __queue_cap_release causes kernel panic.

The mds now sends back a created inode if the create request
performed the create. If the file already existed, no inode is
returned in the reply. This allows ceph to set the created flag
in atomic_open so that permissions are properly checked in the case
that the file wasn't created by the create call to the mds.
To ensure compability with previous kernels, a feature for sending
back the inode in the create reply was added, so that the mds will
only send back the inode if the client indicates it supports the
feature.

The error returned by ceph_mdsc_do_request includes errors sending the
request, errors on timeout, or any errors coming from the mds. If
ceph_mdsc_do_request returns an error, the reply struct will most likely
be bogus. We need to bail out and propogate the error instead of
overwriting it.

writeback_inodes_sb(_nr)_if_idle() is re-implemented by replacing down_read()
with down_read_trylock() because
- If ->s_umount is write locked, then the sb is not idle. That is
writeback_inodes_sb(_nr)_if_idle() needn't wait for the lock.
- writeback_inodes_sb(_nr)_if_idle() grabs s_umount lock when it want to start
writeback, it may bring us deadlock problem when doing umount. In order to
fix the problem, ext4 and btrfs implemented their own writeback functions
instead of writeback_inodes_sb(_nr)_if_idle(), but it introduced the redundant
code, it is better to implement a new writeback_inodes_sb(_nr)_if_idle().
The name of these two functions is cumbersome, so rename them to
try_to_writeback_inodes_sb(_nr).
This idea came from Christoph Hellwig.

"Below are the core block IO bits for 3.9. It was delayed a few days
since my workstation kept crashing every 2-8h after pulling it into
current -git, but turns out it is a bug in the new pstate code (divide
by zero, will report separately). In any case, it contains:
- The big cfq/blkcg update from Tejun and and Vivek.
- Additional block and writeback tracepoints from Tejun.
- Improvement of the should sort (based on queues) logic in the plug
flushing.
- _io() variants of the wait_for_completion() interface, using

blkdev_ioctl(GETBLKSIZE) uses i_size_read() to read size of block device.
If we update block size directly, reader may see intermediate result in
some machines and configurations. Use i_size_write() instead.

Add tracepoints for page dirtying, writeback_single_inode start, inode
dirtying and writeback. For the latter two inode events, a pair of
events are defined to denote start and end of the operations (the
starting one has _start suffix and the one w/o suffix happens after
the operation is complete). These inode ops are FS specific and can
be non-trivial and having enclosing tracepoints is useful for external
tracers.
This is part of tracepoint additions to improve visiblity into
dirtying / writeback operations for io tracer and userland.
v2: writeback_dirty_inode[_start] TPs may be called for files on
pseudo FSes w/ unregistered bdi. Check whether bdi->dev is %NULL
before dereferencing.

We want to add a trace point to touch_buffer() but macros and inline
functions defined in header files can't have tracing points. Move
touch_buffer() to fs/buffer.c and make it a proper function.
The new exported function is also declared inline. As most uses of
touch_buffer() are inside buffer.c with nilfs2 as the only other user,
the effect of this change should be negligible.

bio completion didn't kick block_bio_complete TP. Only dm was
explicitly triggering the TP on IO completion. This makes
block_bio_complete TP useless for tracers which want to know about
bios, and all other bio based drivers skip generating blktrace
completion events.
This patch makes all bio completions via bio_endio() generate
block_bio_complete TP.
* Explicit trace_block_bio_complete() invocation removed from dm and
the trace point is unexported.
* @rq dropped from trace_block_bio_complete(). bios may fly around
w/o queue associated. Verifying and accessing the assocaited queue
belongs to TP probes.
* blktrace now gets both request and bio completions. Make it ignore
bio completions if request completion path is happening.
This makes all bio based drivers generate blktrace completion events
properly and makes the block_bio_complete TP actually useful.
v2: With this change, block_bio_complete TP could be invoked on sg

I'm not sure why, but the hlist for each entry iterators were conceived
list_for_each_entry(pos, head, member)
The hlist ones were greedy and wanted an extra parameter:
hlist_for_each_entry(tpos, pos, head, member)
Why did they need an extra pos parameter? I'm not quite sure. Not only
they don't really need it, it also prevents the iterator from looking
exactly like the list iterator, which is unfortunate.
Besides the semantic patch, there was some manual work required:
- Fix up the actual hlist iterators in linux/list.h
- Fix up the declaration of other iterators based on the hlist ones.
- A very small amount of places were using the 'node' parameter, this
was modified to use 'obj->member' instead.
- Coccinelle didn't handle the hlist_for_each_entry_safe iterator
properly, so those had to be fixed up manually.

MAX_IDR_MASK is another weirdness in the idr interface. As idr covers
whole positive integer range, it's defined as 0x7fffffff or INT_MAX.
Its usage in idr_find(), idr_replace() and idr_remove() is bizarre.
They basically mask off the sign bit and operate on the rest, so if
the caller, by accident, passes in a negative number, the sign bit
will be masked off and the remaining part will be used as if that was
the input, which is worse than crashing.
The constant is visible in idr.h and there are several users in the
kernel.
* drivers/i2c/i2c-core.c:i2c_add_numbered_adapter()
Basically used to test if adap->nr is a negative number which isn't
-1 and returns -EINVAL if so. idr_alloc() already has negative
@start checking (w/ WARN_ON_ONCE), so this can go away.
* drivers/infiniband/core/cm.c:cm_alloc_id()
drivers/infiniband/hw/mlx4/cm.c:id_map_alloc()
Used to wrap cyclic @start. Can be replaced with max(next, 0).
Note that this type of cyclic allocation using idr is buggy. These
are prone to spurious -ENOSPC failure after the first wraparound.
* fs/super.c:get_anon_bdev()
The ID allocated from ida is masked off before being tested whether
it's inside valid range. ida allocated ID can never be a negative
number and the masking is unnecessary.
Update idr_*() functions to fail with -EINVAL when negative @id is
specified and update other MAX_IDR_MASK users as described above.
This leaves MAX_IDR_MASK without any user, remove it and relocate
other MAX_IDR_* constants to lib/idr.c.

Note that the adhoc cyclic id allocation is buggy. If wraparound
happens, the previous code with idr_get_new_above() may segfault and
the converted code will trigger WARN and return -EINVAL. Even if it's
fixed to wrap to zero, the code will be prone to unnecessary -ENOSPC
failures after the first wraparound. We probably need to implement
proper cyclic support in idr.

idr_destroy() can destroy idr by itself and idr_remove_all() is being
deprecated.
The conversion isn't completely trivial for recover_idr_clear() as it's
the only place in kernel which makes legitimate use of idr_remove_all()
w/o idr_destroy(). Replace it with idr_remove() call inside
idr_for_each_entry() loop. It goes on top so that it matches the
operation order in recover_idr_del().

Convert recover_idr_clear() to use idr_for_each_entry() instead of
idr_for_each(). It's somewhat less efficient this way but it shouldn't
matter in an error path. This is to help with deprecation of
idr_remove_all().

There is no documented methods to mark FAT as dirty. Unofficially MS
started to use reserved Byte in boot sector for this purpose, at least
since Win 2000. With Win 7 user is warned if fs is dirty and asked to
clean it.
Different versions of Win, handle it in different ways, but always have
same meaning:
- Win 2000 and XP, set it on write operations and
remove it after operation was finnished
- Win 7, set dirty flag on first write and remove it on umount.
We will do it as follows:
- set dirty flag on mount. If fs was initially dirty, warn user,
remember it and do not do any changes to boot sector.
- clean it on umount. If fs was initially dirty, leave it dirty.
- do not do any thing if fs mounted read-only.
- TODO: leave fs dirty if we found some error after mount.

The fsck_hfs (under MacOS X) complains about unzeroed unused b-tree nodes
after deletion of folders' tree under Linux.
SYMPTOMS:
Running Disk Utiltiy's "Verify Disk" on "test" gives the following:
Verifying volume “Test”
Checking file systemChecking Journaled HFS Plus volume.
Checking extents overflow file.
Checking catalog file.
Unused node is not erased (node = 3111)
Checking multi-linked files.
Checking catalog hierarchy.
Checking extended attributes file.
Checking volume bitmap.
Checking volume information.
The volume Test was found corrupt and needs to be repaired.
Error: This disk needs to be repaired. Click Repair Disk.
REPRODUCING PATH:
1. Prepare HFS+ (non-case sensitive) partition (for example, 5GB)
under MacOS X.
2. Copy linux kernel source tree (for example, 3.7-rc6 version) on
this partition under MacOS X.
3. Then switch to Linux and mount this prepared partition.
4. Execute `sudo rm -r` under prepared directory with linux kernel
source tree.
5. Unmount and boot back into OS X.
6. Open up Disk Utility and verify partition.
REPRODUCIBILITY: 100%
FIX:
It is added code of node clearing in hfs_bnode_put() method for the case
when node has flag HFS_BNODE_DELETED.

ocfs2_block_group_alloc_discontig() disables chain relink by setting
ac->ac_allow_chain_relink = 0 because it grabs clusters from multiple
cluster groups.
It doesn't keep the credits for all chain relink,but
ocfs2_claim_suballoc_bits overrides this in this call trace:
ocfs2_block_group_claim_bits()->ocfs2_claim_clusters()->
__ocfs2_claim_clusters()->ocfs2_claim_suballoc_bits()
ocfs2_claim_suballoc_bits set ac->ac_allow_chain_relink = 1; then call
ocfs2_search_chain() one time and disable it again, and then we run out
of credits.
Fix is to allow relink by default and disable it in
ocfs2_block_group_alloc_discontig.
Without this patch, End-users will run into a crash due to run out of
credits, backtrace like this:

We need to re-initialize the security for a new reflinked inode with its
parent dirs if it isn't specified to be preserved for ocfs2_reflink().
However, the code logic is broken at ocfs2_init_security_and_acl()
although ocfs2_init_security_get() succeed. As a result,
ocfs2_acl_init() does not involked and therefore the default ACL of
parent dir was missing on the new inode.
Note this was introduced by 9d8f13ba3 ("security: new
security_inode_init_security API adds function callback")
To reproduce:
set default ACL for the parent dir(ocfs2 in this case):

default ACL entry(user:jeff:rwx)was missing:

In this case, the end user can also observed an error message at syslog:

This fixes a regression introduced by commit f7fec032aa782. The
problem was that the extents status flags caused us to mask out block
numbers smaller than 2**28 blocks. Since we didn't test with file
systems smaller than 512GB, we didn't notice this during the
development cycle.
A typical failure looks like this:
EXT4-fs error (device sdb1): htree_dirblock_to_tree:919: inode #172235804: block
152052301: comm ls: bad entry in directory: rec_len is smaller than minimal -
offset=0(0), inode=0, rec_len=0, name_len=0
... where 'debugfs -R "stat <172235804>" /dev/sdb1' reports that the
inode has block number 688923213. When viewed in hex, block number
152052301 (from the syslog) is 0x910224D, while block number 688923213
is 0x2910224D. Note the missing "0x20000000" in the block number.
