This fixes the problem that a cooling device may be referenced by
by multiple trip points in multiple thermal zones.
With this patch, we have two stages for updating a thermal zone,
1. check if a thermal_instance needs to be updated or not
2. update the cooling device, based on the target cooling state
of all its instances.
Note that, currently, the cooling device is set to the deepest
cooling state required.

This function is used to update the cooling state of
all the cooling devices that are bound to an active trip point.
This will be used for passive cooling as well, in the future patches.
as both active and passive cooling can share the same algorithm,
which is
1. if the temperature is higher than a trip point,
a. if the trend is THERMAL_TREND_RAISING, use higher cooling
state for this trip point
b. if the trend is THERMAL_TREND_DROPPING, use lower cooling
state for this trip point
2. if the temperature is lower than a trip point, use lower
cooling state for this trip point.

According to ACPI spec, tc1 and tc2 are used by OSPM
to anticipate the temperature trends.
We introduced the same concept to the generic thermal layer
for passive cooling, but now it seems that these values
are hard to be used on other platforms.
So We introduce .get_trend() as a more general solution.
For the platform thermal drivers that have their own way to
anticipate the temperature trends, they should provide
their own .get_trend() callback.
Or else, we will calculate the temperature trends by simply
comparing the current temperature and the cached previous
temperature reading.

As the active cooling devices can have multiple cooling states,
we may want only several cooling states for a certain trip point,
and other cooling states for other active trip points.
To do this, we should be able to describe the cooling device
behavior for a certain trip point, rather than for the entire thermal zone.
And when updating thermal zone, we need to check the upper and lower limit
to make sure the cooling device is set to the proper cooling state.
Note that this patch will not bring any different behavior as
upper limit is set to max_state and lower limit is set to 0
in this patch, for now.
Next patch will set these to real values.

The Linux EC driver includes a mechanism to detect GPE storms,
and switch from interrupt-mode to polling mode. However, polling
mode sometimes doesn't work, so the workaround is problematic.
Also, different systems seem to need the threshold for detecting
the GPE storm at different levels.
ACPI_EC_STORM_THRESHOLD was initially 20 when it's created, and
was changed to 8 in 2.6.28 commit 06cf7d3c7 "ACPI: EC: lower interrupt storm
threshold" to fix kernel bug 11892 by forcing the laptop in that bug to
work in polling mode. However in bug 45151, it works fine in interrupt
mode if we lift the threshold back to 20.
This patch makes the threshold a module parameter so that user has a
flexible option to debug/workaround this issue.
The default is unchanged.

Analysis:
Commit 3d339dc (cpuidle / ACPI : move cpuidle_device field out of the
acpi_processor_power structure()) made the allocation of the dev structure
(struct cpuidle) of a CPU dynamic, whereas previously it was statically
allocated. And this dynamic allocation occurs in acpi_processor_power_init()
if pr->flags.power evaluates to non-zero.
On KVM guests, pr->flags.power evaluates to zero, hence dev is never
allocated. This causes the NULL pointer (dev) dereference in
cpuidle_disable_device() during a subsequent CPU online operation. Fix this
by ensuring that dev is non-NULL before dereferencing.

It is possible to miss data when using the kdb pager. The kdb pager
does not pay attention to the maximum column constraint of the screen
or serial terminal. This result is not incrementing the shown lines
correctly and the pager will print more lines that fit on the screen.
Obviously that is less than useful when using a VGA console where you
cannot scroll back.
The pager will now look at the kdb_buffer string to see how many
characters are printed. It might not be perfect considering you can
output ASCII that might move the cursor position, but it is a
substantially better approximation for viewing dmesg and trace logs.
This also means that the vt screen needs to set the kdb COLUMNS
variable.

It is a common enough mistake for people to specify "kdb" when they
meant to type "kbd" that the kgdboc can just accept both since they
both mean the same thing anyway. Specifically it is for the case
where you want kdb to be active on your graphics console + keyboard
(where kbd was the original abbreviation for keyboard).
With this change kgdboc will now accept either to mean the same thing:
kgdboc=kbd
kgdboc=kdb

Use the recently-added bio front_pad field to allocate struct dm_target_io.
Prior to this patch, dm_target_io was allocated from a mempool. For each
dm_target_io, there is exactly one bio allocated from a bioset.
This patch merges these two allocations into one allocation: we create a
bioset with front_pad equal to the size of dm_target_io so that every
bio allocated from the bioset has sizeof(struct dm_target_io) bytes
before it. We allocate a bio and use the bytes before the bio as
dm_target_io.
_tio_cache is removed and the tio_pool mempool is now only used for
request-based devices.
This idea was introduced by Kent Overstreet.

Support discards when the pool's block size is not a power of 2.
The block layer assumes discard_granularity is a power of 2 (in
blkdev_issue_discard), so we set this to the largest power of 2 that is
a divides into the number of sectors in each block, but never less than
DATA_DEV_BLOCK_SIZE_MIN_SECTORS.
This patch eliminates the "Discard support must be disabled when the
block size is not a power of 2" constraint that was imposed in commit
55f2b8b ("dm thin: support for non power of 2 pool blocksize"). That
commit was incomplete: using a block size that is not a power of 2
shouldn't mean disabling discard support on the device completely.

Use the ACCESS_ONCE macro in dm-bufio and dm-verity where a variable
can be modified asynchronously (through sysfs) and we want to prevent
compiler optimizations that assume that the variable hasn't changed.

Properly account for I/O in transit before returning from the RESET call.
In the absense of this patch, we could have a situation where the host may
respond to a command that was issued prior to the issuance of the RESET
command at some arbitrary time after responding to the RESET command.
Currently, the host does not do anything with the RESET command.

Currently it is possible to unmap one more block than user requested to
due to the off-by-one error in unmap_region(). This is probably due to
the fact that the end variable despite its name actually points to the
last block to unmap + 1. However in the condition it is handled as the
last block of the region to unmap.
The bug was not previously spotted probably due to the fact that the
region was not zeroed, which has changed with commit
be1dd78de5686c062bb3103f9e86d444a10ed783. With that commit we were able
to corrupt the ext4 file system on 256M scsi_debug device with LBPRZ
enabled using fstrim.
Since the 'end' semantic is the same in several functions there this
commit just fixes the condition to use the 'end' variable correctly in
that context.

This patch tries to shorten the path length of scsi_cmd_to_driver(). As only
REQ_TYPE_BLOCK_PC commands can be submitted without a driver, so we could
avoid the related NULL checking, as long as we make sure we don't use it for
REQ_TYPE_BLOCK_PC type commands. Plus, this fixes a bug where you get
different behaviors from REQ_TYPE_BLOCK_PC commands when a driver is and isn't
attached.

Support the LUN parameter change event. Currently, the host fires this event
when the capacity of a disk is changed from the virtual machine monitor.

virtio-scsi needs to report LUNs greater than 256 using the "flat"
format. Because the Linux SCSI layer just maps the SCSI LUN to
an u32, without any parsing, these end up in the range from 16640
to 32767. Fix max_lun to account for the possibility that logical
unit numbers are encoded with the "flat" format.
