Allocate the data for the GID_PN request through the new FC
kmem_cache. While updating the GID_PN code, also introduce a helper
function for initializing the CT header for FC nameserver requests.
Remove the "paranoia" check as well, the GID_PN request data does not
suddenly change.

A data buffer that is passed to the hardware must not cross a page
boundary. zfcp uses a series of kmem_caches to align the data to not
cross a page boundary. Introduce a new kmem_cache for the FC requests
sent from the zfcp driver and use it for the ELS ADISC data. The goal
is to migrate to the FC kmem_cache in later patches and remove the
request specific kmem_caches.

zfcp requires a mempool for the status read data blocks to resubmit
the "status read" requests at any time. Each status read data block
has the size of a page (4096 bytes) and needs to be placed in one
page.
Instead of having a kmem_cache for allocating page sized chunks, use
mempool_create_page_pool to create a mempool returning pages and
remove the zfcp kmem_cache.

If a qeth device is set offline, data and control subchannels are
cleared, which means removal of all IP Assist Primitive settings
implicitly. There is no need to delete those settings explicitly.
This patch removes all IP Assist invocations from offline.

The 34xx and 3590 tape driver uses the system work queue to defer work
from the interrupt function to process context, e.g. a medium sense
after an unsolicited interrupt. The tape commands started by the work
handler need to be asynchronous, otherwise a deadlock on the system
work queue can occur.

The "ct" variable should be an unsigned int. Both struct kbdiacrs
->kb_cnt and struct kbd_data ->accent_table_size are unsigned ints.
Making it signed causes a problem in KBDIACRUC because the user could
set the signed bit and cause a buffer overflow.

The module parameter 'devs' and 'sizes' are marked as __initdata. The
memory for the parameters are freed after module_init completed. This
can lead to kernel crashes in param_free_charp. Remove the __initdata
attribute to fix the problem.

as function argument returns the size of pointer.
sizeof(ipuser) did not result in 16 (as many programmers would have
expected) but sizeof(u8 *), so it is 4 or 8, too small here.

OSA device is in suspend state. This patch enables proper
resuming of an OSA device in this case.

A framesize-change may occur while a Linux system with probed
HiperSockets device is in suspend state. This patch enables proper

Setting of a MAC-address may fail because an already used MAC-address
is to bet set or because of authorization problems. In those cases
qeth issues a message, but the mentioned MAC-address is not the
new MAC-address to be set, but the actual MAC-address. This patch
chooses now the new MAC-address to be set for the error messages.

If a device is not resumed correctly the system crashes when this
device is set offline. This may happen if it gets disconnected
during suspend.
Check if the device is already removed from alias handling and skip
these steps to prevent the kernel panic.

The open function of qeth is not executed if the qeth device is in
state DOWN or HARDSETUP. A recovery switches from state SOFTSETUP to
HARDSETUP to DOWN to HARDSETUP and back to SOFTSETUP. If open and
recover are running concurrently, open fails if it hits the states
HARDSETUP or DOWN. This patch inserts waiting for recovery finish
in the qeth open functions to enable successful qeth device opening
in spite of a running recovery.

Over time, block layer has accumulated a set of APIs dealing with bdev
open, close, claim and release.
* blkdev_get/put() are the primary open and close functions.
* bd_claim/release() deal with exclusive open.
* open/close_bdev_exclusive() are combination of open and claim and
the other way around, respectively.
* bd_link/unlink_disk_holder() to create and remove holder/slave
symlinks.
* open_by_devnum() wraps bdget() + blkdev_get().
The interface is a bit confusing and the decoupling of open and claim
makes it impossible to properly guarantee exclusive access as
in-kernel open + claim sequence can disturb the existing exclusive
open even before the block layer knows the current open if for another
exclusive access. Reorganize the interface such that,
* blkdev_get() is extended to include exclusive access management.
@holder argument is added and, if is @FMODE_EXCL specified, it will
gain exclusive access atomically w.r.t. other exclusive accesses.
* blkdev_put() is similarly extended. It now takes @mode argument and
if @FMODE_EXCL is set, it releases an exclusive access. Also, when
the last exclusive claim is released, the holder/slave symlinks are
removed automatically.
* bd_claim/release() and close_bdev_exclusive() are no longer
necessary and either made static or removed.
* bd_link_disk_holder() remains the same but bd_unlink_disk_holder()
is no longer necessary and removed.
* open_bdev_exclusive() becomes a simple wrapper around lookup_bdev()
and blkdev_get(). It also has an unexpected extra bdev_read_only()
test which probably should be moved into blkdev_get().
* open_by_devnum() is modified to take @holder argument and pass it to
blkdev_get().
Most of bdev open/close operations are unified into blkdev_get/put()
and most exclusive accesses are tested atomically at the open time (as
it should). This cleans up code and removes some, both valid and
invalid, but unnecessary all the same, corner cases.
open_bdev_exclusive() and open_by_devnum() can use further cleanup -
rename to blkdev_get_by_path() and blkdev_get_by_devt() and drop
special features. Well, let's leave them for another day.
Most conversions are straight-forward. drbd conversion is a bit more
involved as there was some reordering, but the logic should stay the
same.
