ASID allocation changes/1
This patch does 2 things:
(1) get_new_mmu_context() NOW moves mm->ASID to a new value ONLY if it
was from a prev allocation cycle/generation OR if mm had no ASID
allocated (vs. before would unconditionally moving to a new ASID)
Callers desiring unconditional update of ASID, e.g.local_flush_tlb_mm()
(for parent's address space invalidation at fork) need to first force
the parent to an unallocated ASID.
(2) get_new_mmu_context() always sets the MMU PID reg with unchanged/new
ASID value.
The gains are:
- consolidation of all asid alloc logic into get_new_mmu_context()
- avoiding code duplication in switch_mm() for PID reg setting
- Enables future change to fold activate_mm() into switch_mm()

With previous commit freeing up PTE bits, reassign them so as to:
- Match the bit to H/w counterpart where possible
(e.g. MMUv2 GLOBAL/PRESENT, this avoids a shift in create_tlb())

The current ARC VM code has 13 flags in Page Table entry: some software
(accesed/dirty/non-linear-maps) and rest hardware specific. With 8k MMU
page, we need 19 bits for addressing page frame so remaining 13 bits is
just about enough to accomodate the current flags.
In MMUv4 there are 2 additional flags, SZ (normal or super page) and WT
(cache access mode write-thru) - and additionally PFN is 20 bits (vs. 19
before for 8k). Thus these can't be held in current PTE w/o making each
entry 64bit wide.
It seems there is some scope of compressing the current PTE flags (and
freeing up a few bits). Currently PTE contains fully orthogonal distinct
access permissions for kernel and user mode (Kr, Kw, Kx; Ur, Uw, Ux)
which can be folded into one set (R, W, X). The translation of 3 PTE
bits into 6 TLB bits (when programming the MMU) can be done based on
following pre-requites/assumptions:
1. For kernel-mode-only translations (vmalloc: 0x7000_0000 to
0x7FFF_FFFF), PTE additionally has PAGE_GLOBAL flag set (and user
space entries can never be global). Thus such a PTE can translate
to Kr, Kw, Kx (as appropriate) and zero for User mode counterparts.
2. For non global entries, the PTE flags can be used to create mirrored
K and U TLB bits. This is true after commit a950549c675f2c8c504
"ARC: copy_(to|from)_user() to honor usermode-access permissions"
which ensured that user-space translations _MUST_ have same access
permissions for both U/K mode accesses so that copy_{to,from}_user()
play fair with fault based CoW break and such...
There is no such thing as free lunch - the cost is slightly infalted
TLB-Miss Handlers.

In the exception return path, for both U/K cases, intr are already
disabled (for various existing reasons). So when we drop down to
@restore_regs, we need not redo that.
There was subtle issue - when intr were NOT being disabled for
ret-to-kernel-but-no-preemption case - now fixed by moving the
IRQ_DISABLE further up in @resume_kernel_mode.
So what do we gain:
* Shaves off a few insn in return path.
* Eliminates the need for IRQ_DISABLE_SAVE assembler macro for ARCv2
hence allows for entry code sharing.

After the recent cleanups, all the exception handlers now have same
boilerplate prologue code. Move that into common macro.
This reduces readability but helps greatly with sharing / duplicating
entry code with ARCv2 ISA where the handlers are pretty much the same,
just the entry prologue is different (due to hardware assist).
Also while at it, add the missing FAKE_RET_FROM_EXCPN calls in couple of
places to drop down to pure kernel mode (from exception mode) before
jumping off into "C" code.

There is no reason we cannot use the generic iomap support to give us
the ioread* and iowrite* family of IO access functions. The m68k arch with
MMU enabled does, so this makes us consistent for all m68k now.
Some potentially valid drivers will fail to compile without these,
for example:

When the signal stack frame is created, it must be flushed in order to
make sure the cache fetches the correct data.
Without cache flush the icache might pick up old cached data from an older
signal stack frame if the signal is raised again very fast.
In case of copyback the data cache muist be pushed first, but is untested.

If UML is not run by a shell it can happen that UML
will kill unrelated proceses upon a fatal exit because
it issues a kill(0, ...).
To prevent such oddities we create a new session in main().

Richard reported that some UML processes survive if the UML
main process receives a SIGTERM.
This issue was caused by a wrongly placed signal(SIGTERM, SIG_DFL)
in init_new_thread_signals().
It disabled the UML exit handler accidently for some processes.
The correct solution is to disable the fatal handler for all
UML helper threads/processes.
Such that last_ditch_exit() does not get called multiple times
and all processes can exit due to SIGTERM.

UML's block device driver does not support write barriers,
to support this this patch adds REQ_FLUSH suppport.
Every time the block layer sends a REQ_FLUSH we fsync() now
our backing file to guarantee data consistency.

UML needs it's own probe_kernel_read() to handle kernel
mode faults correctly.
The implementation uses mincore() on the host side to detect
whether a page is owned by the UML kernel process.
This fixes also a possible crash when sysrq-t is used.
Starting with 3.10 sysrq-t calls probe_kernel_read() to
read details from the kernel workers. As kernel worker are
completely async pointers may turn NULL while reading them.

This affects xen pv guests with sufficiently old versions of xen and
sufficiently new hardware. On such a system, a guest with a btrfs

Pull ARM SoC board updates from Olof Johansson:
"Board updates for 3.12. Again, a bit of domain overlap with SoC and
DT branches, but most of this is around legacy code and board support.
We've found that platform maintainers have a hard time separating all
of these out and might move towards fewer branches for next release.
- Removal of a number of Marvell Kirkwood board files, since contents
is now common and mostly configured via DT.
- Device-tree updates for Marvell Dove, including irqchip and
clocksource setup.
- Defconfig updates. Gotta go somewhere. One new one for Renesas
Lager.
- New backlight drivers for backlights used on Renesas shmobile
platforms.
- Removal of Renesas leds driver.
- Shuffling of some of the new Broadcom platforms to give room for
others in the same mach directory. More in 3.13"

The current bcm_kona_smc_init function throws a BUG_ON if there's no SMC
device node defined in the device tree.
Since secure API access is optional depending the chip configuration,
fix this by allowing the rest of the code to run even if there's no SMC
device node defined

This patchset renames all uses of "bcm," name bindings to
"brcm," as they were done prior to knowing that brcm had
already been standardized as Broadcom vendor prefix
(in Documentation/devicetree/bindings/vendor-prefixes.txt).
This will not cause any churn on devices because none of
these bindings have made it into production yet.

Some Armada 370/XP boards have GPIO buttons, so it makes sense to
enable gpio-keys and the input event interface in mvebu_defconfig.

Various kirkwood boards make use of i2c devices. Ensure the defconfig
builds them. Also, EXT4 FS is also common on NAS boxes, so include it
as well. And lastly, NSA310 has a RealTek 8169 onboard.

This adds an initial DT file for the Globalscale D2Plug with Dove SoC.
Currently, one LED is missing and I have not been able to get SD8787 driver
working. Those will be taken care of later.
