Use proc_create()/proc_create_data() to make sure that ->proc_fops and ->data
be setup before gluing PDE to main tree.
Add correct ->owner to proc_fops to fix reading/module unloading race.

Printk was observed to hang during module unload due to a limited
window of characters that may be sent to the hypervisor. The window
only reexpands when we receive an ack from the HV and the spinlock here
prevents us from ever processing that ack. This fixes it by dropping
the lock before doing the printk, then looping back to the top to
reacquire the lock.

Commit fbd568a3e61a7decb8a754ad952aaa5b5c82e9e5 ("Change
synchronize_kernel to _rcu and _sched") changed the deprecated
synchronize_kernel() in HvLpEvent_unregisterHandler() to
synchronize_rcu(). It turns out that it should have been
synchronize_sched().

Many struct file_operations in the kernel can be "const". Marking them const
moves these to the .rodata section, which avoids false sharing with potential
dirty data. In addition it'll catch accidental writes at compile time to
these shared resources.

Remove struct pt_regs * from all handlers.
Also remove the regs argument from get_irq() functions.
Compile tested with arch/powerpc/config/* and
arch/ppc/configs/prep_defconfig

When I changed the hvlpevent_queue code to use a spinlock instead of a
custom atomic (719d1cd86780c156f954fc34f34481adac197aec) I didn't
initialise the lock anywhere, oops.

A comment in lpevents.c refers to code that's actually in HvCallEvent.h.
The code in HvCallEvent.h is pretty obvious, so just remove the comment
altogether.
