In the patch "aio: convert the ioctx list to table lookup v3", incorrect
handling in the ioctx_alloc() error path was introduced that lead to an
ioctx being added via ioctx_add_table() while freed when the ioctx_alloc()
call returned -EAGAIN due to hitting the aio_max_nr limit. Fix this by
only calling ioctx_add_table() as the last step in ioctx_alloc().
Also, several unnecessary rcu_dereference() calls were added that lead to
RCU warnings where the system was already protected by a spin lock for
accessing mm->ioctx_table.

In the event that an overflow/underflow occurs while calculating req_batch,
clamp the minimum at 1 request instead of doing a BUG_ON().

I've got an alternate approach for fixing this wart in lookup_ioctx()...
Instead of using an rbtree, just use the reserved id in the ring buffer
header to index an array pointing the ioctx. It's not finished yet, and
it needs to be tidied up, but is most of the way there.

With the changes to use percpu counters for aio event ring size calculation,
existing increases to aio_max_nr are now insufficient to allow for the
allocation of enough events. Double the value used for aio_max_nr to account
for the doubling introduced by the percpu slack.

The kiocb refcount is only needed for cancellation - to ensure a kiocb
isn't freed while a ki_cancel callback is running. But if we restrict
ki_cancel callbacks to not block (which they currently don't), we can
simply drop the refcount.

The old aio retry infrastucture needed to save the various arguments to
to aio operations. But with the retry infrastructure gone, we can trim
struct kiocb quite a bit.

This code doesn't serve any purpose anymore, since the aio retry
infrastructure has been removed.
This change should be safe because aio_read/write are also used for
synchronous IO, and called from do_sync_read()/do_sync_write() - and
there's no looping done in the sync case (the read and write syscalls).

aio_complete() (arguably) needs to keep its own trusted copy of the tail
pointer, but io_getevents() doesn't have to use it - it's already using
the head pointer from the ring buffer.
So convert it to use the tail from the ring buffer so it touches fewer
cachelines and doesn't contend with the cacheline aio_complete() needs.

Originally, io_event() was documented to return the io_event if
cancellation succeeded - the io_event wouldn't be delivered via the ring
buffer like it normally would.
But this isn't what the implementation was actually doing; the only
driver implementing cancellation, the usb gadget code, never returned an
io_event in its cancel function. And aio_complete() was recently changed
to no longer suppress event delivery if the kiocb had been cancelled.
This gets rid of the unused io_event argument to kiocb_cancel() and
kiocb->ki_cancel(), and changes io_cancel() to return -EINPROGRESS if
kiocb->ki_cancel() returned success.
Also tweak the refcounting in kiocb_cancel() to make more sense.

See the previous patch ("aio: reqs_active -> reqs_available") for why we
want to do this - this basically implements a per cpu allocator for
reqs_available that doesn't actually allocate anything.
Note that we need to increase the size of the ringbuffer we allocate,
since a single thread won't necessarily be able to use all the
reqs_available slots - some (up to about half) might be on other per cpu
lists, unavailable for the current thread.
We size the ringbuffer based on the nr_events userspace passed to
io_setup(), so this is a slight behaviour change - but nr_events wasn't
being used as a hard limit before, it was being rounded up to the next
page before so this doesn't change the actual semantics.

The number of outstanding kiocbs is one of the few shared things left that
has to be touched for every kiocb - it'd be nice to make it percpu.
We can make it per cpu by treating it like an allocation problem: we have
a maximum number of kiocbs that can be outstanding (i.e. slots) - then we
just allocate and free slots, and we know how to write per cpu allocators.
So as prep work for that, we convert reqs_active to reqs_available.

When "fs/aio: Add support to aio ring pages migration" was applied, it
broke the build when CONFIG_MIGRATION was disabled. Wrap the migration
code with a test for CONFIG_MIGRATION to fix this and save a few bytes
when migration is disabled.

As the aio job will pin the ring pages, that will lead to mem migrated
failed. In order to fix this problem we use an anon inode to manage the aio ring
pages, and setup the migratepage callback in the anon inode's address space, so
that when mem migrating the aio ring pages will be moved to other mem node safely.

Introduce a new lib function anon_inode_getfile_private(), it creates a new file
instance by hooking it up to an anonymous inode, and a dentry that describe the
"class" of the file, similar to anon_inode_getfile(), but each file holds a
single inode. Furthermore, anyone who wants to create a private anon file will
benefit from this change.

Unfortunately, even with a softdep some distros fail to include
the necessary modules in the initrd. Therefore this patch adds
a fallback path to restore existing behaviour where we cannot
load the new crypto crct10dif algorithm.
In order to do this, the underlying crct10dif has been split out
from the crypto implementation so that it can be used on the
fallback path.

crypto_larval_lookup should only return a larval if it created one.
Any larval created by another entity must be processed through
crypto_larval_wait before being returned.
Otherwise this will lead to a larval being killed twice, which
will most likely lead to a crash.

Rearranging code so that crashk_res gets updated.
- crashk_res is updated after mips_parse_crashkernel(),
after resource_init(), which is after arch_mem_init().
- The reserved memory is actually treated as Usable memory,
Unless we load the crash kernel, everything works.

This change corrects DECstation HRT calibration, by removing the following
bugs:
1. Calibration period selection -- HZ / 10 has been chosen, however on
DECstation computers, HZ never divides by 10, as the choice for HZ is
among 128, 256 and 1024. The choice therefore results in a systematic
calibration error, e.g. 6.25% for the usual choice of 128 for HZ:
128 / 10 * 10 = 120
(128 - 120) / 128 -> 6.25%
The change therefore makes calibration use HZ / 8 that is always
accurate for the HZ values available, getting rid of the systematic
error.
2. Calibration starting point synchronisation -- the duration of a number
of intervals between DS1287A periodic interrupt assertions is measured,
however code does not ensure at the beginning that the interrupt has
not been previously asserted. This results in a variable error of e.g.
up to another 6.25% for the period of HZ / 8 (8.(3)% with the original
HZ / 10 period) and the usual choice of 128 for HZ:
1 / 16 -> 6.25%
1 / 12 -> 8.(3)%
The change therefore adds an initial call to ds1287_timer_state that
clears any previous periodic interrupt pending.
The same issue applies to both I/O ASIC counter and R4k CP0 timer
calibration on DECstation systems as similar code is used in both cases
and both pieces of code are covered by this fix.
On an R3400 test system used this fix results in a change of the I/O ASIC
clock frequency reported from values like:
I/O ASIC clock frequency 23185830Hz
to:
I/O ASIC clock frequency 24999288Hz
removing the miscalculation by 6.25% from the systematic error and (for
the individual sample provided) a further 1.00% from the variable error,
accordingly. The nominal I/O ASIC clock frequency is 25MHz on this
system.
Here's another result, with the fix applied, from a system that has both
HRTs available (using an R4400 at 60MHz nominal):
MIPS counter frequency 59999328Hz
I/O ASIC clock frequency 24999432Hz

The NR_CPUS_DEFAULT_1 introduced as an aid for the QEMU
platform in 72ede9b18967e7a8a62a88f164f003193f6d891f
"[MIPS] Qemu: Fix Symmetric Uniprocessor support"
which was later removed in
302922e5f6901eb6f29c58539631f71b3d9746b8
"[MIPS] Qemu: Remove platform."
On certain randconfigs it may happen for NR_CPUS to have an
empty value because not all SMP platforms select a suitable
NR_CPUS_DEFAULT_* value. We fix this by restoring the range
of NR_CPUS to 2..64 and drop the NR_CPUS_DEFAULT_{1,2} symbols.
The first one is no longer used and the latter is not needed since
NR_CPUS=2 is now the default value.
Fixes the following problem on a randconfig:

Dynamic function tracing was not working on MIPS. When doing dynamic
tracing, the tracer attempts to match up the passed in address with
the one the compiler creates in the mcount tables. The MIPS code was
passing in the return address from the tracing function call, but the
compiler tables were the address of the function call. So they
wouldn't match.
Just subtracting 8 from the return address will give the address of
the function call. Easy enough.

This is a fix for a bug introduced with commit
447cdf2628b59aa513a42785450b348dced26d8a, submitted as archived here:

regrettably with no further explanation.
The issue is with the CP0 Count register read erratum present on R4000 and
some R4400 processors. If this erratum is present, then a read from this
register that happens around the time it reaches the value stored in the
CP0 Compare register causes a CP0 timer interrupt that is supposed to
happen when the values in the two registers match to be missed. The
implication for the chips affected is the CP0 timer can be used either as
a source of a timer interrupt (a clock event) or as a source of a
high-resolution counter (a clock source), but not both at a time.
The erratum does not affect timer interrupt operation itself, because in
this case the CP0 Count register is only read while the timer interrupt
has already been raised, while high-resolution counter references happen
at random times.
Additionally some systems apparently have issues with the timer interrupt
line being routed externally and not following the usual CP0 Count/Compare
semantics. In this case we don't want to use the R4k clock event.
We've meant to address the erratum and the timer interrupt routing issue
in time_init, however the commit referred to above broke our solution.
What we currently have is we enable the R4k clock source if the R4k clock
event initialization has succeeded (the timer is present and has no timer
interrupt routing issue) or there is no CP0 Count register read erratum.
Which gives the following boolean matrix:

What we want instead is to enable the R4k clock source if there is no CP0
Count register read erratum (obviously) or the R4k clock event
initialization has *failed* -- because in the latter case we won't be
using the timer interrupt anyway, so we don't care about any interference
CP0 Count reads might cause with the interrupt. This corresponds to the
following boolean matrix:

This is implemented here, effectively reverting the problematic commit,
and a short explanation is given next to code modified so that the
rationale is known to future readers and confusion is prevented from
happening here again.
It is worth noting that mips_clockevent_init returns 0 upon success while
cpu_has_mfc0_count_bug returns 0 upon failure. This is because the former
function returns an error code while the latter returns a boolean value.
To signify the difference I have therefore chosen to compare the result of
the former call explicitly against 0.

get_user_pages_fast() is missing cache flushes for MIPS platforms with
cache aliases. Filesystem failures observed with DirectIO operations due
to missing flush_anon_page() that use page coloring logic to work with
cache aliases. This fix falls through to take slow_irqon path that calls
get_user_pages() that has required logic for platforms where
cpu_has_dc_aliases is true.

The Ralink RT3883 SoCs have a built-in PCI Host Controller
device. The patch adds a platform driver and device tree
binding documentation for that.
The patch also enables the HW_HAS_PCI config option. This
is required in order to be able to enable the PCI support.

The BMIPS5000 (Zephyr) processor utilizes instruction speculation. A
stale misprediction address in either the JTB or the CRS may trigger
a prefetch inside a region that is currently being used by a DMA engine,
which is not IO-coherent. This prefetch will fetch a line into the
scache, and that line will soon become stale (ie wrong) during/after the
DMA. Mayhem ensues.
In dma-default.c, the r10000 is handled as a special case in the same way
that we want to handle Zephyr. So we generalize the exception cases into
a function, and include Zephyr as one of the processors that needs this
special care.
