[SCSI] qla2xxx: Cruft cleanup of functions and structures.
Strip unused (DEBUG-ONLY) enabled functions, inlines, useless wrappers, and unused DPC flags from the code. Another step in the migration towards a cleaner (less-crusty) driver.
