IB/mthca: Clean up mthca array index mask
Define a constant MTHCA_ARRAY_MASK to replace repeated uses of (PAGE_SIZE / sizeof (void *) - 1) in mthca array code. Signed-off-by: Roland Dreier <rolandd@cisco.com>
