KVM: MMU: More struct kvm_vcpu -> struct kvm cleanups
This time, the biggest change is gpa_to_hpa. The translation of GPA to HPA does
not depend on the VCPU state unlike GVA to GPA so there's no need to pass in
the kvm_vcpu.
