Pull device mapper updates from Mike Snitzer:

- a DM thin-provisioning block discard vs allocation race fix that
addresses potential for corruption
- a stable fix for DM verity-fec's block calculation during decode

These only work if the we use the same initiator ID for all path,
which might not be true if we use different protocols, or even just
different HBAs.

I wish the OSD code could simply use blk_rq_map_* helpers like
everyone else, but the complex nature of deciding if we have
DATA IN and/or DATA OUT buffers might make this impossible
(at least for a mere human like me).
But using blk_rq_append_bio at least allows sharing the setup code
between request with or without dat a buffers, and given that this
is the last user of blk_make_request it allows getting rid of that
somewhat awkward interface.

This adds a REQ_OP_FLUSH operation that is sent to request_fn
based drivers by the block layer's flush code, instead of
sending requests with the request->cmd_flags REQ_FLUSH bit set.

This patch drops the compat definition of req_op where it matches
the rq_flag_bits definitions, and drops the related old and compat
code that allowed users to set either the op or flags for the operation.
We also then store the operation in the bi_rw/cmd_flags field similar

The req operation REQ_OP is separated from the rq_flag_bits
definition. This converts the block layer drivers to
use req_op to get the op from the request struct.

This patch converts the simple bi_rw use cases in the block,
drivers, mm and fs code to set/get the bio operation using
bio_set_op_attrs/bio_op
These should be simple one or two liner cases, so I just did them
in one patch. The next patches handle the more complicated
cases in a module per patch.

We currently set REQ_WRITE/WRITE for all non READ IOs
like discard, flush, writesame, etc. In the next patches where we
no longer set up the op as a bitmap, we will not be able to
detect a operation direction like writesame by testing if REQ_WRITE is
set.
This patch converts the drivers and cgroup to use the
op_is_write helper. This should just cover the simple
cases. I did dm, md and bcache in their own patches
because they were more involved.

iscsi_sw_tcp_data_ready() and iscsi_sw_tcp_state_change() were
using read_lock(&sk->sk_callback_lock) which is fine if caller
disabled BH.
TCP stack no longer has this requirement and can run from
process context.
Use read_lock_bh() variant to restore previous assumption.
Ideally this code could use RCU instead...

Commit b704f70ce200 ("SCSI: fix bug in scsi_dev_info_list matching")
changed the way vendor- and model-string matching was carried out in the
routine that looks up entries in a SCSI devinfo list. The new matching
code failed to take into account the case of a maximum-length string; in
such cases it could end up testing for a terminating '\0' byte beyond
the end of the memory allocated to the string. This out-of-bounds bug
was detected by UBSAN.
I don't know if anybody has actually encountered this bug. The symptom
would be that a device entry in the blacklist might not be matched
properly if it contained an 8-character vendor name or a 16-character
model name. Such entries certainly exist in scsi_static_device_list.
This patch fixes the problem by adding a check for a maximum-length
string before the '\0' test.

If we fall back to using LSI on the Croc or Crocodile chip we need to

The untagged command case in the 53c700 driver has been broken since
host wide tags were enabled because the replaced scsi_find_tag()
function had a special case for the tag value SCSI_NO_TAG to retrieve
sdev->current_cmnd. The replacement function scsi_host_find_tag() has
no such special case and returns NULL causing untagged commands to
trigger a BUG() in the driver. Inspection shows that the 53c700 is the
only driver using this SCSI_NO_TAG case, so a local fix in the driver
suffices to fix this problem globally.

sas_ata_strategy_handler() adds the works of the ata error handler to
system_unbound_wq. This workqueue asynchronously runs work items, so the
ata error handler will be performed concurrently on different CPUs. In
this case, ->host_failed will be decreased simultaneously in
scsi_eh_finish_cmd() on different CPUs, and become abnormal.
It will lead to permanently inequality between ->host_failed and
->host_busy, and scsi error handler thread won't start running. IO
errors after that won't be handled.
Since all scmds must have been handled in the strategy handler, just
remove the decrement in scsi_eh_finish_cmd() and zero ->host_busy after
the strategy handler to fix this race.

For historic reasons, io_opt is in bytes and max_sectors in block layer
sectors. This interface inconsistency is error prone and should be
fixed. But for 4.4--4.7 let's make the unit difference explicit via a
wrapper function.

Only SRC-based adapters support the AifReqEvent function, so there is no
point in trying to activate it on older, non-SRC based adapters. Doing
so lead to crashes on older adapters.

sd_check_events() is called asynchronously, and might race
with device removal. So always take a disk reference when
processing the event to avoid the device being removed while
the event is processed.
