Right now the Makefile for the mpt3sas driver does not even allow the
driver to be built into the kernel. So fix that up, as there doesn't
seem to be any obvious reason why this shouldn't be done.

This fixes an issue seen with devices getting marked offline
in a scenario where a VIOS was getting rebooted while a
client VFC adapter is in SCSI EH and prevents unnecessary
EH escalation in some scenarios.

The kmalloc* functions of all slab allcoators are similar now so
lets move them into slab.h. This requires some function naming changes
in slob.
As a results of this patch there is a common set of functions for
all allocators. Also means that kmalloc_large() is now available
in general to perform large order allocations that go directly
via the page allocator. kmalloc_large() can be substituted if
kmalloc() throws warnings because of too large allocations.
kmalloc_large() has exactly the same semantics as kmalloc but
can only used for allocations > PAGE_SIZE.

If we have multiple sessions on a system, we normally don't want
background sessions to read input events. Otherwise, it could capture
passwords and more entered by the user on the foreground session. This is
a real world problem as the recent XMir development showed:

We currently rely on sessions to release input devices when being
deactivated. This relies on trust across sessions. But that's not given on
usual systems. We therefore need a way to control which processes have
access to input devices.
With VTs the kernel simply routed them through the active /dev/ttyX. This
is not possible with evdev devices, though. Moreover, we want to avoid
routing input-devices through some dispatcher-daemon in userspace (which
would add some latency).
This patch introduces EVIOCREVOKE. If called on an evdev fd, this revokes
device-access irrecoverably for that *single* open-file. Hence, once you
call EVIOCREVOKE on any dup()ed fd, all fds for that open-file will be
rather useless now (but still valid compared to close()!). This allows us
to pass fds directly to session-processes from a trusted source. The
source keeps a dup()ed fd and revokes access once the session-process is
no longer active.
Compared to the EVIOCMUTE proposal, we can avoid the CAP_SYS_ADMIN
restriction now as there is no way to revive the fd again. Hence, a user
is free to call EVIOCREVOKE themself to kill the fd.
Additionally, this ioctl allows multi-layer access-control (again compared
to EVIOCMUTE which was limited to one layer via CAP_SYS_ADMIN). A middle
layer can simply request a new open-file from the layer above and pass it
to the layer below. Now each layer can call EVIOCREVOKE on the fds to
revoke access for all layers below, at the expense of one fd per layer.
There's already ongoing experimental user-space work which demonstrates
how it can be used:

Matt found that commit 27a7c642174e ("partitions/efi: account for pmbr
size in lba") caused his GPT formatted eMMC device not to boot. The
reason is that this commit enforced Linux to always check the lesser of
the whole disk or 2Tib for the pMBR size in LBA. While most disk
partitioning tools out there create a pMBR with these characteristics,
Microsoft does not, as it always sets the entry to the maximum 32-bit
limitation - even though a drive may be smaller than that[1].
Loosen this check and only verify that the size is either the whole disk
or 0xFFFFFFFF. No tool in its right mind would set it to any value
other than these.

It is very likely that block device inode will be part of BDI dirty list
as well. However it doesn't make sence to sort inodes on the b_io list
just because of this inode (as it contains buffers all over the device
anyway). So save some CPU cycles which is valuable since we hold relatively
contented wb->list_lock.

The LRU list changes interacted badly with our nr_dentry_unused
accounting, and even worse with the new DCACHE_LRU_LIST bit logic.
This introduces helper functions to make sure everything follows the
proper dcache d_lru list rules: the dentry cache is complicated by the
fact that some of the hotpaths don't even want to look at the LRU list
at all, and the fact that we use the same list entry in the dentry for
both the LRU list and for our temporary shrinking lists when removing
things from the LRU.
The helper functions temporarily have some extra sanity checking for the
flag bits that have to match the current LRU state of the dentry. We'll
remove that before the final 3.12 release, but considering how easy it
is to get wrong, this first cleanup version has some very particular
sanity checking.

Commits 925f5532 (xtensa: ccount based clockevent implementation) and e3f43291
(xtensa: ccount based sched_clock) introduced users of ccount_freq. This
variable doesn't exist when CONFIG_XTENSA_CALIBRATE_CCOUNT is disabled. Add
ccount_freq definition in this case.

-e is not needed to output strings without escape sequences. This breaks
big endian FSF build when the shell is dash, because its builtin echo
doesn't understand '-e' switch and outputs it in the echoed string.

Check pending signals and rescheduling thread flags with interrupts
disabled, and don't enable them if no flags are set. Call
trace_hardirqs_on after thread flags handling, so that rescheduling is
done and hardirqs tracking flag is updated in the correct task context.

Prior to the introduction of page migration support in "fs/aio: Add support
to aio ring pages migration" / 36bc08cc01709b4a9bb563b35aa530241ddc63e3,
mapping of the ring buffer pages was done via get_user_pages() while
retaining mmap_sem held for write. This avoided possible races with userland
racing an munmap() or mremap(). The page migration patch, however, switched
to using mm_populate() to prime the page mapping. mm_populate() cannot be
called with mmap_sem held.
Instead of dropping the mmap_sem, revert to the old behaviour and simply
drop the use of mm_populate() since get_user_pages() will cause the pages to
get mapped anyways. Thanks to Al Viro for spotting this issue.

Sseveral sparse warnings were caused by missing rcu_dereference() annotations
for dereferencing mm->ioctx_table. Thankfully, none of those were actual bugs
as the deref was protected by a spin lock in all instances.

The commit 36bc08cc0170 ("fs/aio: Add support to aio ring pages migration")
added some debugging code that is not required and resulted in a build error
when 98474236f72e ("vfs: make the dentry cache use the lockref infrastructure")
was added to the tree. The code is not required, so just delete it.

Another shortcoming of the table lookup patch was revealed where the pointer
was not being tested before being dereferenced. Verify this to avoid the
NULL pointer dereference.
